Narrative:
Aircraft xyz had a nose gear that would not come down on first attempt. I had a mechanic change nose gear bungee cylinder. After the job was done and all operations checks, leak check and gear swings had been accomplished and verified by required inspection item inspector, I signed the write-up off. On dec/xa/99, aircraft xyz had to use alternate gear extension to get nose gear down. Upon inspection, found a tool used to change the bungee cylinder stuck in gear well.

Title: A DC9-50 NOSE GEAR FAILED TO EXTEND NORMALLY. CREW HAD TO USE MANUAL EXTENSION TO EXTEND GEAR CAUSED BY BUNGEE CYLINDER TOOL LEFT IN EXTEND MECHANISM.
Narrative: ACFT XYZ HAD A NOSE GEAR THAT WOULD NOT COME DOWN ON FIRST ATTEMPT. I HAD A MECH CHANGE NOSE GEAR BUNGEE CYLINDER. AFTER THE JOB WAS DONE AND ALL OPS CHKS, LEAK CHK AND GEAR SWINGS HAD BEEN ACCOMPLISHED AND VERIFIED BY REQUIRED INSPECTION ITEM INSPECTOR, I SIGNED THE WRITE-UP OFF. ON DEC/XA/99, ACFT XYZ HAD TO USE ALTERNATE GEAR EXTENSION TO GET NOSE GEAR DOWN. UPON INSPECTION, FOUND A TOOL USED TO CHANGE THE BUNGEE CYLINDER STUCK IN GEAR WELL.
