Narrative:
Airworthiness directive 91-24-14 check for compliance on a 4 1/2 carbon seal spacer. Spacer failed the rockwell hardness test and was removed from service. At that point it was inadvertently sent to the vendor instead of being sent to the carrier engineering department.

Title: WHILE ACCOMPLISHING AN AIRWORTHINESS DIRECTIVE ON A JT8D LOW PRESSURE TURBINE SPACER THE FAILED PART WAS SENT TO THE VENDOR AND NOT TO THE CARRIER ENGINEERING DEPT AS REQUIRED BY THE AIRWORTHINESS DIRECTIVE.
Narrative: AIRWORTHINESS DIRECTIVE 91-24-14 CHK FOR COMPLIANCE ON A 4 1/2 CARBON SEAL SPACER. SPACER FAILED THE ROCKWELL HARDNESS TEST AND WAS REMOVED FROM SVC. AT THAT POINT IT WAS INADVERTENTLY SENT TO THE VENDOR INSTEAD OF BEING SENT TO THE CARRIER ENGINEERING DEPT.
