Narrative:

Teterboro four departure was flown as published. New york approach gave direct to coate. I did not see coate in the FMS, but typed it in. I questioned the sic as to the route. He said the flight plan was correct. I asked the sic to check if the next shown fix was ewc. Center said no, it was lhy. I typed in lake henry next and asked the sic to take the controls. I reached for the flight plan (gdc) and saw a different route hand written. The sic had the correct transponder code, altitude and frequency in the boxes, but not the correct departure route. Just selected the wrong flight plan. No big turns or vectors, as I questioned the rest of the clearance.

Title: CHARTER CREW EXPERIENCES TRACK DEV DEPARTING TETERBORO DUE TO MISPROGRAMMING OF THE FMS.

Narrative: TETERBORO FOUR DEP WAS FLOWN AS PUBLISHED. NEW YORK APCH GAVE DIRECT TO COATE. I DID NOT SEE COATE IN THE FMS, BUT TYPED IT IN. I QUESTIONED THE SIC AS TO THE RTE. HE SAID THE FLT PLAN WAS CORRECT. I ASKED THE SIC TO CHK IF THE NEXT SHOWN FIX WAS EWC. CTR SAID NO, IT WAS LHY. I TYPED IN LAKE HENRY NEXT AND ASKED THE SIC TO TAKE THE CTLS. I REACHED FOR THE FLT PLAN (GDC) AND SAW A DIFFERENT RTE HAND WRITTEN. THE SIC HAD THE CORRECT XPONDER CODE, ALT AND FREQ IN THE BOXES, BUT NOT THE CORRECT DEP RTE. JUST SELECTED THE WRONG FLT PLAN. NO BIG TURNS OR VECTORS, AS I QUESTIONED THE REST OF THE CLRNC.
