Narrative:
Aircraft X was on a VOR approach practice inbound. Aircraft Y inbound 15 mi from west. It was discovered that aircraft X was passed by aircraft Y right to left by 20 ft over drk VOR. Tower was first informed of this after aircraft X landed. Aircraft Y never saw #1 and was unfamiliar with airport. Aircraft Y was first instructed right traffic runway 21R to report a 2 mi right 45 degree to the downwind, and was first observed on a left base to runway 21L, nearly missing aircraft Z, another aircraft on the left downwind.
