Narrative:
7 pieces of hazmat were loaded in the forward cargo compartment in oma, accompanied by 7 ok-XXXZ for the flight to ZZZ and on to lax -- same flight number and aircraft. After gate arrival at ZZZ, a person came to the cockpit and told me that we might have a live animal from ZZZ to lax and if so, the 7 pieces of hazmat would be moved to the mid cargo compartment with the live animal in the forward. During taxi out, we received the first load closeout and it showed 1 live animal forward and no hazmat in any compartments. The first officer mentioned that he had seen 3 hazmat boxes on the ramp near our aircraft. We called load control and requested a correct closeout. The second closeout showed 1 live animal forward and 10 pieces of hazmat in the mid. We called loads again and requested another closeout and to verify with the loading supervisor for our flight the correct number of pieces of hazmats -- and if there were 10 pieces, then we would taxi back to the gate and get the new ok-XXXZ. The loads person called back and said that there were 7 pieces of hazmat in the mid and a new closeout would be sent. The last closeout showed 1 live animal forward and 7 pieces of hazmat in the mid. After arrival at lax, I witnessed a baggage handler count 10 pieces of hazmat in the mid. We had flown to lax without the correct final closeout for the hazmat and did not have ok-XXXZ for the additional piece. Callback conversation with reporter revealed the following information: the reporter stated the hazmat manifest was correct on departure from omaha and was listed as through ZZZ to lax so we knew at least 7 hazmat containers were on board. The reporter said after pushback the hazmat manifest was found to be in error as no hazmat was listed. It took 2 more attempts with load planning to supposedly get a correct manifest. The reporter stated the third manifest found at the terminating station to be also incorrect. The reporter said this type of hazmat manifest error occurred 2 weeks prior to this event, but the aircraft was brought back to the gate to get correction and a positive hazmat container count.

Title: AN MD SUPER 80 WAS DISPATCHED IN NON COMPLIANCE WITH AN INCORRECT HAZMAT MANIFEST AFTER 3 CREW ATTEMPTS TO SECURE CORRECTION.
Narrative: 7 PIECES OF HAZMAT WERE LOADED IN THE FORWARD CARGO COMPARTMENT IN OMA, ACCOMPANIED BY 7 OK-XXXZ FOR THE FLT TO ZZZ AND ON TO LAX -- SAME FLT NUMBER AND ACFT. AFTER GATE ARR AT ZZZ, A PERSON CAME TO THE COCKPIT AND TOLD ME THAT WE MIGHT HAVE A LIVE ANIMAL FROM ZZZ TO LAX AND IF SO, THE 7 PIECES OF HAZMAT WOULD BE MOVED TO THE MID CARGO COMPARTMENT WITH THE LIVE ANIMAL IN THE FORWARD. DURING TAXI OUT, WE RECEIVED THE FIRST LOAD CLOSEOUT AND IT SHOWED 1 LIVE ANIMAL FORWARD AND NO HAZMAT IN ANY COMPARTMENTS. THE FO MENTIONED THAT HE HAD SEEN 3 HAZMAT BOXES ON THE RAMP NEAR OUR ACFT. THE LOADS PERSON CALLED BACK AND SAID THAT THERE WERE 7 PIECES OF HAZMAT IN THE MID AND A NEW CLOSEOUT WOULD BE SENT. THE LAST CLOSEOUT SHOWED 1 LIVE ANIMAL FORWARD AND 7 PIECES OF HAZMAT IN THE MID. AFTER ARR AT LAX, I WITNESSED A BAGGAGE HANDLER COUNT 10 PIECES OF HAZMAT IN THE MID. WE HAD FLOWN TO LAX WITHOUT THE CORRECT FINAL CLOSEOUT FOR THE HAZMAT AND DID NOT HAVE OK-XXXZ FOR THE ADDITIONAL PIECE. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE HAZMAT MANIFEST WAS CORRECT ON DEP FROM OMAHA AND WAS LISTED AS THROUGH ZZZ TO LAX SO WE KNEW AT LEAST 7 HAZMAT CONTAINERS WERE ON BOARD. THE RPTR SAID AFTER PUSHBACK THE HAZMAT MANIFEST WAS FOUND TO BE IN ERROR AS NO HAZMAT WAS LISTED. IT TOOK 2 MORE ATTEMPTS WITH LOAD PLANNING TO SUPPOSEDLY GET A CORRECT MANIFEST.
