Narrative:

A flight attendant was injured because she did not follow instructions to remain seated. Once on the ground, severe turbulence description was written in the logbook. I had to use two separate log pages to write the narrative. When the mechanics completed the inspection, they only signed off one of the log pages and we only discovered they missed the other, when we were airborne on our return to phx. However, the second page was referenced to the previous page via log number. Maintenance supervisor signed off the second page in phx.

Title: PLT RPT, B737. SEVERE TURB. CAPT TOLD CABIN ATTENDANT TO REMAIN SEATED. NON ADHERENCE. CABIN ATTENDANT INJURY. LAND IN PUEBLO. MAINT INSPECTION. LOG SIGN OFF, ERROR.

Narrative: A FLT ATTENDANT WAS INJURED BECAUSE SHE DID NOT FOLLOW INSTRUCTIONS TO REMAIN SEATED. ONCE ON THE GND, SEVERE TURB DESCRIPTION WAS WRITTEN IN THE LOGBOOK. I HAD TO USE TWO SEPARATE LOG PAGES TO WRITE THE NARRATIVE. WHEN THE MECHS COMPLETED THE INSPECTION, THEY ONLY SIGNED OFF ONE OF THE LOG PAGES AND WE ONLY DISCOVERED THEY MISSED THE OTHER, WHEN WE WERE AIRBORNE ON OUR RETURN TO PHX. HOWEVER, THE SECOND PAGE WAS REFED TO THE PREVIOUS PAGE VIA LOG NUMBER. MAINT SUPVR SIGNED OFF THE SECOND PAGE IN PHX.
