Narrative:
On departure from lax, currently on a vector, off SID, then socal told us to proceed direct seal beach, then cleared the transition to flight plan route. Smo was tuned in vors and our initial turn was to smo (heading of northwest at the time). Very soon after rolling out, socal repeated that we were cleared sli, and directed a turn towards sli. Tune, identify, and monitor! Supplemental information from acn 464503: several factors caused error: 1) takeoff procedure required use of smo VOR. 2) FMS dropped sli as waypoint after takeoff, so FMS/INS was not usable. 3) delaying vectors and hold-downs increased workload. 4) IFR conditions made visual awareness of wrong direction impossible. Supplemental information from acn 464659: flew by mistake towards smo VOR which was still dialed in from the departure procedure.
Original NASA ASRS Text
Title: AN L1011 FLC FLIES TOWARDS THE WRONG VOR DURING THEIR DEP PROC S OF LAX, CA.
Narrative: ON DEP FROM LAX, CURRENTLY ON A VECTOR, OFF SID, THEN SOCAL TOLD US TO PROCEED DIRECT SEAL BEACH, THEN CLRED THE TRANSITION TO FLT PLAN RTE. SMO WAS TUNED IN VORS AND OUR INITIAL TURN WAS TO SMO (HDG OF NW AT THE TIME). VERY SOON AFTER ROLLING OUT, SOCAL REPEATED THAT WE WERE CLRED SLI, AND DIRECTED A TURN TOWARDS SLI. TUNE, IDENT, AND MONITOR! SUPPLEMENTAL INFO FROM ACN 464503: SEVERAL FACTORS CAUSED ERROR: 1) TKOF PROC REQUIRED USE OF SMO VOR. 2) FMS DROPPED SLI AS WAYPOINT AFTER TKOF, SO FMS/INS WAS NOT USABLE. 3) DELAYING VECTORS AND HOLD-DOWNS INCREASED WORKLOAD. 4) IFR CONDITIONS MADE VISUAL AWARENESS OF WRONG DIRECTION IMPOSSIBLE. SUPPLEMENTAL INFO FROM ACN 464659: FLEW BY MISTAKE TOWARDS SMO VOR WHICH WAS STILL DIALED IN FROM THE DEP PROC.
