Narrative:

Aircraft xyz (B727) on a flight from ZZZ (flight XXX) rptedly lost a tailcone on takeoff out of ZZZ. Maintenance control (abc) called to inspect engines to determine if a tailcone was missing. On preliminary inspection it was determined that #2 engine was missing a tailcone and a gouge was located in the acoustical liner. 2 mechanics were assigned to determine damage and in working with approved manuals the tailcone was deferred. An engineering repair authority/authorized wxwx was approved to apply a time limited repair to the acoustical liner. Any other damage was reported to be minimal. The tailcone entry and engineering repair authority/authorized entry were logged and per entries the airworthiness release was signed.

Title: A B727-200 ON TKOF LOST #2 ENG TAILCONE AND DAMAGED THE ACOUSTICAL TAILPIPE LINER. DAMAGE INSPECTED AND WITH ENGINEERING AUTH DEFERRED.

Narrative: ACFT XYZ (B727) ON A FLT FROM ZZZ (FLT XXX) RPTEDLY LOST A TAILCONE ON TKOF OUT OF ZZZ. MAINT CTL (ABC) CALLED TO INSPECT ENGS TO DETERMINE IF A TAILCONE WAS MISSING. ON PRELIMINARY INSPECTION IT WAS DETERMINED THAT #2 ENG WAS MISSING A TAILCONE AND A GOUGE WAS LOCATED IN THE ACOUSTICAL LINER. 2 MECHS WERE ASSIGNED TO DETERMINE DAMAGE AND IN WORKING WITH APPROVED MANUALS THE TAILCONE WAS DEFERRED. AN ENGINEERING REPAIR AUTH WXWX WAS APPROVED TO APPLY A TIME LIMITED REPAIR TO THE ACOUSTICAL LINER. ANY OTHER DAMAGE WAS RPTED TO BE MINIMAL. THE TAILCONE ENTRY AND ENGINEERING REPAIR AUTH ENTRY WERE LOGGED AND PER ENTRIES THE AIRWORTHINESS RELEASE WAS SIGNED.
