Narrative:

During climb out from abc (to cde) the flight attendant observed a lot of water spewing out of the right coffee maker onto the galley and galley floor. He then notified us. Abc maintenance informed us that cde maintenance would meet us upon our arrival. They decided to drain the water and defer the coffee maker. We did depart late from abc (approximately 30 mins) due to an unrelated maintenance write-up, so we in turn were running late for our departure from cde. I (the captain) meant to make myself a written reminder to put on my yoke clip so as not to forget the maintenance clipboard, but was distracted by other duties and neglected to do so. Meanwhile cde maintenance had taken the clipboard off the aircraft and out of my view to finish the paperwork. The ground personnel were anxious to send us on our way and, in our haste, we departed without the clipboard. This could have been avoided if the mechanic had not taken the clipboard off the airplane or at least finished his paperwork in my view. The coffee maker was deferred and the safety of flight was not an issue. Although I try not to let it affect me, there is a lot of pressure and intimidation tactics coming from our management to improve on time performance and this does put oneself in a 'hurry-up' mode (basic human factors). The company should not pressure people unnecessarily when safety is involved. Supplemental information from acn 471360: at cde maintenance took the maintenance record from the plane to write up the deferral because passenger were boarding. Maintenance also drained the galley water, then came aboard and gave us the ok to depart, but failed to return the maintenance record. We flew the return flight from cde to abc without the maintenance record onboard the plane. I believe the problem was caused by pressure from company management on pilots to go-go-go at all costs go. We are in negotiations for a contract, and management has been sending us accusing letters to our homes and our company mailboxes, saying some pilots are trying to 'slow down' the airline. The president of the company even sent mail to my home concerning our 'lack-luster performance.' the company also started a policy of asking for employee numbers for mechanical write-ups in hopes of intimidating the pilot group. The previous flight was delayed because baggage handlers broke a cargo door guide which was non deferrable. It seemed as though another broken item, even water gushing out into the airplane might have us both in trouble with the chief pilot. This event could have been avoided. Company management needs to know that broken is broken, and we need to do the right thing even when it's not convenient. Also, the maintenance can should be located where both pilots can see it. (I cannot see it.) also the maintenance record should stay on the plane, and passenger should not be climbing on or over maintenance personnel, which could interfere with the quality of their work, as it did this time. (Hold boarding till maintenance is finished.)

Title: ACR HAS MECHANICAL PROB REPAIRED, BUT DEPARTS WITHOUT THE MAINT LOG. LABOR MGMNT RELATIONS MAY BE THE PRIME DISTR.

Narrative: DURING CLBOUT FROM ABC (TO CDE) THE FLT ATTENDANT OBSERVED A LOT OF WATER SPEWING OUT OF THE R COFFEE MAKER ONTO THE GALLEY AND GALLEY FLOOR. HE THEN NOTIFIED US. ABC MAINT INFORMED US THAT CDE MAINT WOULD MEET US UPON OUR ARR. THEY DECIDED TO DRAIN THE WATER AND DEFER THE COFFEE MAKER. WE DID DEPART LATE FROM ABC (APPROX 30 MINS) DUE TO AN UNRELATED MAINT WRITE-UP, SO WE IN TURN WERE RUNNING LATE FOR OUR DEP FROM CDE. I (THE CAPT) MEANT TO MAKE MYSELF A WRITTEN REMINDER TO PUT ON MY YOKE CLIP SO AS NOT TO FORGET THE MAINT CLIPBOARD, BUT WAS DISTRACTED BY OTHER DUTIES AND NEGLECTED TO DO SO. MEANWHILE CDE MAINT HAD TAKEN THE CLIPBOARD OFF THE ACFT AND OUT OF MY VIEW TO FINISH THE PAPERWORK. THE GND PERSONNEL WERE ANXIOUS TO SEND US ON OUR WAY AND, IN OUR HASTE, WE DEPARTED WITHOUT THE CLIPBOARD. THIS COULD HAVE BEEN AVOIDED IF THE MECH HAD NOT TAKEN THE CLIPBOARD OFF THE AIRPLANE OR AT LEAST FINISHED HIS PAPERWORK IN MY VIEW. THE COFFEE MAKER WAS DEFERRED AND THE SAFETY OF FLT WAS NOT AN ISSUE. ALTHOUGH I TRY NOT TO LET IT AFFECT ME, THERE IS A LOT OF PRESSURE AND INTIMIDATION TACTICS COMING FROM OUR MGMNT TO IMPROVE ON TIME PERFORMANCE AND THIS DOES PUT ONESELF IN A 'HURRY-UP' MODE (BASIC HUMAN FACTORS). THE COMPANY SHOULD NOT PRESSURE PEOPLE UNNECESSARILY WHEN SAFETY IS INVOLVED. SUPPLEMENTAL INFO FROM ACN 471360: AT CDE MAINT TOOK THE MAINT RECORD FROM THE PLANE TO WRITE UP THE DEFERRAL BECAUSE PAX WERE BOARDING. MAINT ALSO DRAINED THE GALLEY WATER, THEN CAME ABOARD AND GAVE US THE OK TO DEPART, BUT FAILED TO RETURN THE MAINT RECORD. WE FLEW THE RETURN FLT FROM CDE TO ABC WITHOUT THE MAINT RECORD ONBOARD THE PLANE. I BELIEVE THE PROB WAS CAUSED BY PRESSURE FROM COMPANY MGMNT ON PLTS TO GO-GO-GO AT ALL COSTS GO. WE ARE IN NEGOTIATIONS FOR A CONTRACT, AND MGMNT HAS BEEN SENDING US ACCUSING LETTERS TO OUR HOMES AND OUR COMPANY MAILBOXES, SAYING SOME PLTS ARE TRYING TO 'SLOW DOWN' THE AIRLINE. THE PRESIDENT OF THE COMPANY EVEN SENT MAIL TO MY HOME CONCERNING OUR 'LACK-LUSTER PERFORMANCE.' THE COMPANY ALSO STARTED A POLICY OF ASKING FOR EMPLOYEE NUMBERS FOR MECHANICAL WRITE-UPS IN HOPES OF INTIMIDATING THE PLT GROUP. THE PREVIOUS FLT WAS DELAYED BECAUSE BAGGAGE HANDLERS BROKE A CARGO DOOR GUIDE WHICH WAS NON DEFERRABLE. IT SEEMED AS THOUGH ANOTHER BROKEN ITEM, EVEN WATER GUSHING OUT INTO THE AIRPLANE MIGHT HAVE US BOTH IN TROUBLE WITH THE CHIEF PLT. THIS EVENT COULD HAVE BEEN AVOIDED. COMPANY MGMNT NEEDS TO KNOW THAT BROKEN IS BROKEN, AND WE NEED TO DO THE RIGHT THING EVEN WHEN IT'S NOT CONVENIENT. ALSO, THE MAINT CAN SHOULD BE LOCATED WHERE BOTH PLTS CAN SEE IT. (I CANNOT SEE IT.) ALSO THE MAINT RECORD SHOULD STAY ON THE PLANE, AND PAX SHOULD NOT BE CLBING ON OR OVER MAINT PERSONNEL, WHICH COULD INTERFERE WITH THE QUALITY OF THEIR WORK, AS IT DID THIS TIME. (HOLD BOARDING TILL MAINT IS FINISHED.)
