Narrative:
I accepted the handoff on aircraft X descending to 14000 ft. Air carrier Y was handed off to lex approach at 11000 ft. When conflict alert activated, I broadcasted for aircraft X to see if he was on frequency since he had not checked in. Aircraft X acknowledged and I asked him to say altitude. He stated he was out of 12000 ft for 11000 ft. I cleared aircraft X to climb and maintain 13000 ft in an attempt to maintain separation. Separation was lost. The controller who originally cleared aircraft Y to descend and maintain 14000 ft had missed a readback error. Aircraft X acknowledged a clearance to 11000 ft. I am not certain but I believe the possible underlying cause of this error to be mental fatigue. Supplemental information from acn 477253: aircraft X was issued descent to 14000 ft. Aircraft X read back 11000 ft. I failed to catch readback error.

Title: A ZID CTLR AT THE PREVIOUS SECTOR MISSED A PLT READBACK ERROR RESULTING IN A LOSS OF SEPARATION IN THE NEXT SECTOR.
Narrative: I ACCEPTED THE HDOF ON ACFT X DSNDING TO 14000 FT. ACR Y WAS HANDED OFF TO LEX APCH AT 11000 FT. WHEN CONFLICT ALERT ACTIVATED, I BROADCASTED FOR ACFT X TO SEE IF HE WAS ON FREQ SINCE HE HAD NOT CHKED IN. ACFT X ACKNOWLEDGED AND I ASKED HIM TO SAY ALT. HE STATED HE WAS OUT OF 12000 FT FOR 11000 FT. I CLRED ACFT X TO CLB AND MAINTAIN 13000 FT IN AN ATTEMPT TO MAINTAIN SEPARATION. SEPARATION WAS LOST. THE CTLR WHO ORIGINALLY CLRED ACFT Y TO DSND AND MAINTAIN 14000 FT HAD MISSED A READBACK ERROR. ACFT X ACKNOWLEDGED A CLRNC TO 11000 FT. I AM NOT CERTAIN BUT I BELIEVE THE POSSIBLE UNDERLYING CAUSE OF THIS ERROR TO BE MENTAL FATIGUE. SUPPLEMENTAL INFO FROM ACN 477253: ACFT X WAS ISSUED DSCNT TO 14000 FT. ACFT X READ BACK 11000 FT.
