Narrative:
Situation happened at mem. Arrived at aircraft, checked logbook with captain. Noted what captain thought was most recent page with discrepancy and sign off. Flew aircraft from mem-iah. When captain completed logbook at iah, he discovered an open write-up underneath cardboard divider that was under what he thought was the last page. Supplemental information from acn 478761: at gate in iah, filling out times in logbook, went to next unused page, found this page to have an open discrepancy which had been written up in memphis. The discrepancy was fixed, but never signed off, and the page it was written on buried under the cardboard underlay. Squared paperwork with maintenance.

Title: A DC9 FLC FAILS TO NOTICE AN OPEN ITEM THAT HAD NOT BEEN SIGNED OFF IN AN UNNOTICED PAGE IN THE LOGBOOK PRIOR TO TKOF AT MEM, TN.
Narrative: SIT HAPPENED AT MEM. ARRIVED AT ACFT, CHKED LOGBOOK WITH CAPT. NOTED WHAT CAPT THOUGHT WAS MOST RECENT PAGE WITH DISCREPANCY AND SIGN OFF. FLEW ACFT FROM MEM-IAH. WHEN CAPT COMPLETED LOGBOOK AT IAH, HE DISCOVERED AN OPEN WRITE-UP UNDERNEATH CARDBOARD DIVIDER THAT WAS UNDER WHAT HE THOUGHT WAS THE LAST PAGE. SUPPLEMENTAL INFO FROM ACN 478761: AT GATE IN IAH, FILLING OUT TIMES IN LOGBOOK, WENT TO NEXT UNUSED PAGE, FOUND THIS PAGE TO HAVE AN OPEN DISCREPANCY WHICH HAD BEEN WRITTEN UP IN MEMPHIS. THE DISCREPANCY WAS FIXED, BUT NEVER SIGNED OFF, AND THE PAGE IT WAS WRITTEN ON BURIED UNDER THE CARDBOARD UNDERLAY. SQUARED PAPERWORK WITH MAINT.
