Narrative:

An error in programming the onboard navigation computer failed to install the kaddy intersection between bld and pgs vors. The display routing showed direct between the 2 transmitters. Both pilots checked and missed the error. Airborne, ATC did not miss the mistake, called the flight with a vector to the right of the programmed error routing. More attention is necessary when programming and checking the routing. This was the second flight of the day after good rest and days off. Supplemental information from acn 479460: we loaded the mead 2 departure in the FMC and it did not have kaddy intersection in it.

Title: FMS PROGRAMMING ERROR RESULTS IN COURSE DEV NEAR LAS VEGAS, NV.

Narrative: AN ERROR IN PROGRAMMING THE ONBOARD NAV COMPUTER FAILED TO INSTALL THE KADDY INTXN BTWN BLD AND PGS VORS. THE DISPLAY ROUTING SHOWED DIRECT BTWN THE 2 XMITTERS.
