Narrative:
Cleared by ground for pushback. After pushback, ground crew was told to 'stand by' when they asked for brakes and disconnect by first officer (SOP until captain talk to them after he sets brakes). This was a relatively new procedure for our company. Ground crew disconnected anyway before captain gave them the ok to do so. Discussion ensued between captain and first officer about procedure not being followed yet. We then taxied to active, contacted tower for takeoff, and were told we never received a taxi clearance. Captain asked if there would be a problem with this and the tower controller said no. This captain had a habit of beginning to taxi before first officer had actually received taxi clearance. This happened several (2-3) times on this trip. He also had a tendency to 'rush' to get off the ground. Both the captain and the first officer, however, missed getting a taxi clearance other than pushback on this occasion. This was also a quick turn, due to a late departure because of a mechanical.

Title: FLC OF A DC9-30 FAILED TO OBTAIN A TAXI CLRNC PRIOR TO TAXI OUT FOR TKOF.
Narrative: CLRED BY GND FOR PUSHBACK. AFTER PUSHBACK, GND CREW WAS TOLD TO 'STAND BY' WHEN THEY ASKED FOR BRAKES AND DISCONNECT BY FO (SOP UNTIL CAPT TALK TO THEM AFTER HE SETS BRAKES). THIS WAS A RELATIVELY NEW PROC FOR OUR COMPANY. GND CREW DISCONNECTED ANYWAY BEFORE CAPT GAVE THEM THE OK TO DO SO. DISCUSSION ENSUED BTWN CAPT AND FO ABOUT PROC NOT BEING FOLLOWED YET. WE THEN TAXIED TO ACTIVE, CONTACTED TWR FOR TKOF, AND WERE TOLD WE NEVER RECEIVED A TAXI CLRNC. CAPT ASKED IF THERE WOULD BE A PROB WITH THIS AND THE TWR CTLR SAID NO. THIS CAPT HAD A HABIT OF BEGINNING TO TAXI BEFORE FO HAD ACTUALLY RECEIVED TAXI CLRNC. THIS HAPPENED SEVERAL (2-3) TIMES ON THIS TRIP. HE ALSO HAD A TENDENCY TO 'RUSH' TO GET OFF THE GND. BOTH THE CAPT AND THE FO, HOWEVER, MISSED GETTING A TAXI CLRNC OTHER THAN PUSHBACK ON THIS OCCASION. THIS WAS ALSO A QUICK TURN, DUE TO A LATE DEP BECAUSE OF A MECHANICAL.
