Narrative:
Complete failure of piston rod seal and retainer. Total loss of hydraulic fluid in l-hand system. 3/4 of center system failure during taxi out. Aircraft return to gate. Improper torque on the gland nut may be the cause of failure. New notes have been added to our overhaul service manual. The gland nut is also sent out to non destructive testing for checks of cracks.

Title: A B757-200, ON TAXI OUT, EXPERIENCED LOSS OF L HYD SYS PRESSURE AND QUANTITY DUE TO A FLT SPOILER ACTUATOR FAILURE.
Narrative: COMPLETE FAILURE OF PISTON ROD SEAL AND RETAINER. TOTAL LOSS OF HYD FLUID IN L-HAND SYS. 3/4 OF CTR SYS FAILURE DURING TAXI OUT. ACFT RETURN TO GATE. IMPROPER TORQUE ON THE GLAND NUT MAY BE THE CAUSE OF FAILURE. NEW NOTES HAVE BEEN ADDED TO OUR OVERHAUL SVC MANUAL. THE GLAND NUT IS ALSO SENT OUT TO NON DESTRUCTIVE TESTING FOR CHKS OF CRACKS.
