Narrative:
The B747 was cleared for a visual approach and did not turn quickly enough. The aircraft crossed through his final and approached a B737 that was making an ILS on a parallel runway. The B737 responded to a TCASII alert and descended to avoid the B747. The B737 later filed an near midair collision. The B747 pilot was charged with a pilot deviation. The approach feeder controller was charged with an operational error for not ensuring the B747 had the ATIS (the ATIS contained a warning that 'simultaneous approachs to parallel runways are in use').
Original NASA ASRS Text
Title: NMAC BTWN A B747 OVERSHOOTING FINAL DURING VISUAL APCH TO PARALLEL RWYS AND A B737 ON FINAL TO THE OTHER PARALLEL RWY.
Narrative: THE B747 WAS CLRED FOR A VISUAL APCH AND DID NOT TURN QUICKLY ENOUGH. THE ACFT CROSSED THROUGH HIS FINAL AND APCHED A B737 THAT WAS MAKING AN ILS ON A PARALLEL RWY. THE B737 RESPONDED TO A TCASII ALERT AND DSNDED TO AVOID THE B747. THE B737 LATER FILED AN NMAC. THE B747 PLT WAS CHARGED WITH A PLTDEV. THE APCH FEEDER CTLR WAS CHARGED WITH AN OPERROR FOR NOT ENSURING THE B747 HAD THE ATIS (THE ATIS CONTAINED A WARNING THAT 'SIMULTANEOUS APCHS TO PARALLEL RWYS ARE IN USE').
