Narrative:
On a previous leg, sea-sna, the TCASII system failed. We planned on getting an amended release using our company's flight crew placard procedures. Unfortunately, the aircraft did not have any of the required stickers. We contacted maintenance and made the write-up in the logbook. On descent into sna, the TCASII started working. We discussed this with maintenance and elected to call contract maintenance and continue deferring the item since it was now in the logbook. Maintenance signed off the logbook and we departed uneventfully. However, we failed to receive an amended release. Because we were not in the loop with our original idea to use flight crew placarding and because the system was now operating normally, the amended release was very easy to forget.

Title: AN ACR IOE FLT DEPARTS WITHOUT A PROPER RELEASE FROM A NON MAINT STATION.
Narrative: ON A PREVIOUS LEG, SEA-SNA, THE TCASII SYS FAILED. WE PLANNED ON GETTING AN AMENDED RELEASE USING OUR COMPANY'S FLC PLACARD PROCS. UNFORTUNATELY, THE ACFT DID NOT HAVE ANY OF THE REQUIRED STICKERS. WE CONTACTED MAINT AND MADE THE WRITE-UP IN THE LOGBOOK. ON DSCNT INTO SNA, THE TCASII STARTED WORKING. WE DISCUSSED THIS WITH MAINT AND ELECTED TO CALL CONTRACT MAINT AND CONTINUE DEFERRING THE ITEM SINCE IT WAS NOW IN THE LOGBOOK. MAINT SIGNED OFF THE LOGBOOK AND WE DEPARTED UNEVENTFULLY. HOWEVER, WE FAILED TO RECEIVE AN AMENDED RELEASE. BECAUSE WE WERE NOT IN THE LOOP WITH OUR ORIGINAL IDEA TO USE FLC PLACARDING AND BECAUSE THE SYS WAS NOW OPERATING NORMALLY, THE AMENDED RELEASE WAS VERY EASY TO FORGET.
