Narrative:
Between rotation (136 KTS) and 500 ft, experienced severe vibration on #1 engine. Reduced power to no avail. Performed engine out departure, secured #1 engine, and returned to beijing, china runway 36R ILS. Engine #1 turbine section had severe damage with significant metal pieces in exhaust. No visible damage to intake area. Note: to cut through the communications barrier in this part of the world, I consider it mandatory to 'declare an emergency.' the controller understands this and proper service is quickly provided. Callback conversation with reporter revealed the following information: the reporter stated on rotation the vibration from #1 engine was severe and remained so even with the throttle at idle so the engine was shut down. The reporter aid the airplane flew and handled great and the return was routine. The reporter said a preliminary check by local maintenance believes the turbine assembly rear bearing failed. The reporter stated the turbine blades were extensively damaged. Callback conversation with reporter acn 498993 revealed the following information: the reporter stated the vibration after rotation to 500 ft was severe with throttle setting having no effect. The reporter said the engine out on takeoff procedure was performed and engine shut down. The reporter stated on a visual check of the engine the turbine blades were all frayed.
Original NASA ASRS Text
Title: AN MD11 AT ROTATION TO 500 FT DECLARED AN EMER AND DIVERTED DUE TO #1 ENG CONTAINED FAILURE OF THE TURBINE ASSEMBLY.
Narrative: BTWN ROTATION (136 KTS) AND 500 FT, EXPERIENCED SEVERE VIBRATION ON #1 ENG. REDUCED PWR TO NO AVAIL. PERFORMED ENG OUT DEP, SECURED #1 ENG, AND RETURNED TO BEIJING, CHINA RWY 36R ILS. ENG #1 TURBINE SECTION HAD SEVERE DAMAGE WITH SIGNIFICANT METAL PIECES IN EXHAUST. NO VISIBLE DAMAGE TO INTAKE AREA. NOTE: TO CUT THROUGH THE COMS BARRIER IN THIS PART OF THE WORLD, I CONSIDER IT MANDATORY TO 'DECLARE AN EMER.' THE CTLR UNDERSTANDS THIS AND PROPER SVC IS QUICKLY PROVIDED. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED ON ROTATION THE VIBRATION FROM #1 ENG WAS SEVERE AND REMAINED SO EVEN WITH THE THROTTLE AT IDLE SO THE ENG WAS SHUT DOWN. THE RPTR AID THE AIRPLANE FLEW AND HANDLED GREAT AND THE RETURN WAS ROUTINE. THE RPTR SAID A PRELIMINARY CHK BY LCL MAINT BELIEVES THE TURBINE ASSEMBLY REAR BEARING FAILED. THE RPTR STATED THE TURBINE BLADES WERE EXTENSIVELY DAMAGED. CALLBACK CONVERSATION WITH RPTR ACN 498993 REVEALED THE FOLLOWING INFO: THE RPTR STATED THE VIBRATION AFTER ROTATION TO 500 FT WAS SEVERE WITH THROTTLE SETTING HAVING NO EFFECT. THE RPTR SAID THE ENG OUT ON TKOF PROC WAS PERFORMED AND ENG SHUT DOWN. THE RPTR STATED ON A VISUAL CHK OF THE ENG THE TURBINE BLADES WERE ALL FRAYED.
