Narrative:
During routine wheel/brake change, the wheel/axle spacer stuck to the removed brake pack. Technicians failed to notice this and installed new pack and wheel omitting spacer. Aircraft taxied to runway and returned after crew noted noisy (l-hand outboard) wheels/brake. Air crew stopped on taxiway on way back to hangar. Crew got out of aircraft and observed l-hand outboard wheel cocked at an angle. Wheel and tire combination removed from axle (on ramp), extensive damage to wheel/brake pack noted. Aircraft returned to service after repairs. Talking with other technicians, it was found this problem is common knowledge to experienced challenger technicians. Due to the possibility of the spacer sticking to the removed pack and the fact the reinstallation with full functional test is possible suggests 2 possible actions: 1) temporary revision to maintenance manual with an instruction to check for spacer presence. 2) some method of securing spacer to axle shaft to preclude inadvertent removal.

Title: A CANADAIR CL601 WAS DISPATCHED IN NON COMPLIANCE WITH THE L MAIN LNDG GEAR INBOARD WHEEL AND TIRE ASSEMBLY MISSING THE AXLE SPACER.
Narrative: DURING ROUTINE WHEEL/BRAKE CHANGE, THE WHEEL/AXLE SPACER STUCK TO THE REMOVED BRAKE PACK. TECHNICIANS FAILED TO NOTICE THIS AND INSTALLED NEW PACK AND WHEEL OMITTING SPACER. ACFT TAXIED TO RWY AND RETURNED AFTER CREW NOTED NOISY (L-HAND OUTBOARD) WHEELS/BRAKE. AIR CREW STOPPED ON TXWY ON WAY BACK TO HANGAR. CREW GOT OUT OF ACFT AND OBSERVED L-HAND OUTBOARD WHEEL COCKED AT AN ANGLE. WHEEL AND TIRE COMBINATION REMOVED FROM AXLE (ON RAMP), EXTENSIVE DAMAGE TO WHEEL/BRAKE PACK NOTED.
