Narrative:
Right after takeoff, 2 flight attendants from the main cabin rushed to my jump seat. They informed me of a terrible noise in the main cabin. I told them to tell the captain. I proceeded to the main cabin to see what I could hear for input. As I proceeded to main cabin and approached row 13, all of a sudden I saw a panel fly from the ceiling, falling on the floor around the overwing area, then an explosion and what appeared as smoke (but was condensation). When I went to the cockpit to report what I just witnessed, the pilots heard the noise (explosion). I informed them of no serious injuries. Everyone was scared. We went right back to barbados. Entire flight time was 11 mins. We had 2 passenger with minor injuries, from the panel falling on them. On the ground after maintenance checked out the problem, it was determined that ice had formed in the aircraft duct and exploded after takeoff, when full power was on.

Title: B757-200 CREW HAD AIR CONDITIONING DUCT FAILURE DEPARTING TBPB.
Narrative: RIGHT AFTER TKOF, 2 FLT ATTENDANTS FROM THE MAIN CABIN RUSHED TO MY JUMP SEAT. THEY INFORMED ME OF A TERRIBLE NOISE IN THE MAIN CABIN. I TOLD THEM TO TELL THE CAPT. I PROCEEDED TO THE MAIN CABIN TO SEE WHAT I COULD HEAR FOR INPUT. AS I PROCEEDED TO MAIN CABIN AND APCHED ROW 13, ALL OF A SUDDEN I SAW A PANEL FLY FROM THE CEILING, FALLING ON THE FLOOR AROUND THE OVERWING AREA, THEN AN EXPLOSION AND WHAT APPEARED AS SMOKE (BUT WAS CONDENSATION). WHEN I WENT TO THE COCKPIT TO RPT WHAT I JUST WITNESSED, THE PLTS HEARD THE NOISE (EXPLOSION). I INFORMED THEM OF NO SERIOUS INJURIES. EVERYONE WAS SCARED. WE WENT RIGHT BACK TO BARBADOS.
