Narrative:
Amended release not obtained. Copilot's display had 2 off flags. Obtained an en route MEL from maintenance control on taxi out, but not an amended release. (Maintenance could have said 'stand by for dispatch.') I took off without an amended release from dispatch.

Narrative: AMENDED RELEASE NOT OBTAINED. COPLT'S DISPLAY HAD 2 OFF FLAGS. OBTAINED AN ENRTE MEL FROM MAINT CTL ON TAXI OUT, BUT NOT AN AMENDED RELEASE. (MAINT COULD HAVE SAID 'STAND BY FOR DISPATCH.') I TOOK OFF WITHOUT AN AMENDED RELEASE FROM DISPATCH.
