Narrative:

Flight from fll to vdk, routing direct lal VOR direct vdk. Cleared to 4000 ft MSL by ZMA. Handed off to tap approach. On check-in with tpa approach, given direct vdk and immediately issued a 'traffic alert at 1 O'clock, 1 mi, leveloff immediately.' altitude was 4200 ft MSL. PF saw aircraft and initiated evasive maneuver, left 80 degree bank and nose down. Other aircraft passed directly overhead about 100 ft above. Did not see any evasive action by other aircraft. Then advised by tpa approach the aircraft had probably departed lal, and he had just activated his mode C when the traffic alert was issued. I was in the right seat and the aircraft was obscured by my side windshield post. The PF saw traffic through right side windshield and took evasive action. Passenger observed aircraft pass overhead while in steep bank.
