Narrative:

Aircraft #1 was departing cak for sbn. Aircraft #2 was inbound to cak from the wnw. Aircraft #1 was put on a heading to clear cle approach airspace, which put him in conflict with aircraft #2. Aircraft #1 was climbing on a 250 degree heading (on course was at least 280 degrees). Aircraft #2 was descended on approximately 110 degree heading. I forgot that aircraft #2 would turn northeast at dalts, which put him in conflict with aircraft #1. I had already switched aircraft #2 to cak approach when I realized that there was a problem. I turned aircraft #1 on course, but it wasn't enough.
