Narrative:
On jun/sat/01, aircraft X arrived at ZZZ with a PIREP -- no computer on onboard as stated. I deferred the item 'with a lot of assistance from tech' as 'information noted computer is not available at ZZZ, deferred item, technician notified.' for follow-up, entered in sheet, 'circuit breakers pulled and collared.' as crew chief, I assigned 2 mechanics to return the logbook back to the aircraft and pull and collar the circuit breakers. This event occurred just before aircraft departure. With a lot of activity happening in the crew chief office, the 2 mechanics returned and notified me that they could not find the circuit breakers in question. They recalled me stating I would notify maintenance service. I believe this statement to be true. Due to all the activity happening and distrs, I forgot to follow up on the pulling and collaring of the circuit breakers. They also stated I missed entering the PIREP on the sheet. Due to the confusion, I may have missed entering the item on the sheet. The first time I was informed of this incident was on jun/mon/01 at XA30 by certified mail from the FAA.

Title: B777 CREW CHIEF COORDS FOR PLT LOGBOOK ENTRY CORRECTION FOR FAILURE. FAA NOTIFIES CREW CHIEF OF UNCORRECTED WORK 2 WKS LATER.
Narrative: ON JUN/SAT/01, ACFT X ARRIVED AT ZZZ WITH A PIREP -- NO COMPUTER ON ONBOARD AS STATED. I DEFERRED THE ITEM 'WITH A LOT OF ASSISTANCE FROM TECH' AS 'INFO NOTED COMPUTER IS NOT AVAILABLE AT ZZZ, DEFERRED ITEM, TECHNICIAN NOTIFIED.' FOR FOLLOW-UP, ENTERED IN SHEET, 'CIRCUIT BREAKERS PULLED AND COLLARED.' AS CREW CHIEF, I ASSIGNED 2 MECHS TO RETURN THE LOGBOOK BACK TO THE ACFT AND PULL AND COLLAR THE CIRCUIT BREAKERS. THIS EVENT OCCURRED JUST BEFORE ACFT DEP. WITH A LOT OF ACTIVITY HAPPENING IN THE CREW CHIEF OFFICE, THE 2 MECHS RETURNED AND NOTIFIED ME THAT THEY COULD NOT FIND THE CIRCUIT BREAKERS IN QUESTION. THEY RECALLED ME STATING I WOULD NOTIFY MAINT SVC. I BELIEVE THIS STATEMENT TO BE TRUE.
