Narrative:
During takeoff, the pilot failed to notice that the door annunciator light was on. About 45 seconds after takeoff, the door (which was partially latched) popped open and then moved to the full open position. Approximately 10 seconds later, the door separated from the airplane. A normal landing was made and the door was recovered. The damage was nominal and there were no injuries. Callback conversation with reporter revealed the following information: the reporter stated the left gull wing door was opened during taxi for takeoff and the door warning light was illuminated. The reporter said, prior to takeoff, the left door was closed and latched and believed the door warning light was out. The reporter stated on takeoff climb the right gull wing door came open and then departed the aircraft. The reporter said he believed the right door was not positively latched and came open on climb and then departed the aircraft. The reporter said the door was recovered and reinstalled on the airplane.

Title: A COLUMBIA 300 PILOT, ON TKOF CLB, REPORTED THE R PAX GULL WING DOOR DEPARTED THE ACFT.
Narrative: DURING TKOF, THE PLT FAILED TO NOTICE THAT THE DOOR ANNUNCIATOR LIGHT WAS ON. ABOUT 45 SECONDS AFTER TKOF, THE DOOR (WHICH WAS PARTIALLY LATCHED) POPPED OPEN AND THEN MOVED TO THE FULL OPEN POS. APPROX 10 SECONDS LATER, THE DOOR SEPARATED FROM THE AIRPLANE. A NORMAL LNDG WAS MADE AND THE DOOR WAS RECOVERED. THE DAMAGE WAS NOMINAL AND THERE WERE NO INJURIES.
