Narrative:
First officer didn't do job. She put xyz in flight computer which put one route in dispatch, filed another. Dispatch didn't do job. Dispatch put xyz on release, then filed us for a different route. I didn't do job. I didn't ensure first officer did job of comparing automated FMC plan versus dispatch verbiage.
