Narrative:

Failed to make crossing restr. This event occurred during our descent on flight from lax to bna. We failed to make our crossing restr of grahm intersection at 10000 ft. About 400 mi from nashville, we were assigned to fly direct to grahm intersection. It was my leg, and I entered the 10000 ft crossing restr from the STAR into the FMS at that time. I remember noting 'grahm 10000 ft' on the descent page as I scrolled past it. We were subsequently vectored for traffic twice and given a couple of altitude changes, all of which I entered into the FMS. Finally, the controller issued the crossing of grahm at 10000 ft. I descended using the FMS, instead of using my head! At approximately 15-20 mi from grahm intersection the controller asked if we were going to make the restr. I immediately looked down on the descent page and noticed 'grahm, 10000 ft' was no longer there. We asked for and received a turn from ATC, because we were not going to make the restr. We apologized to the controller and asked if there were any conflicts. He advised us there were none, and the rest of the flight was uneventful. This was the last flying leg of my trip, before deadheading back to base. Although this was the fifth consecutive day working, I was not fatigued. This event should have been prevented, if only I would have done what I most certainly always do. When given a crossing restr, I immediately go to the legs page of the FMS to ensure the crossing is entered. More importantly, I always back up myself and the FMS by doing the 'mental math' during the descent. In this case, however, I did neither and relied completely on the FMS instead of using my head. Unfortunately, I am still at a loss as to why I didn't.

Title: XING RESTR ENTRY INTO THE FMS IS REMOVED IN THE COURSE OF SEVERAL PROGRAMMING CHANGES REQUIRED BY ROUTING CHANGE. THE REMOVAL IS NOT NOTICED AND WHEN FINALLY CLRED FOR THE XING, IT IS NOT MADE.

ABOUT 400 MI FROM NASHVILLE, WE WERE ASSIGNED TO FLY DIRECT TO GRAHM INTXN. IT WAS MY LEG, AND I ENTERED THE 10000 FT XING RESTR FROM THE STAR INTO THE FMS AT THAT TIME. I REMEMBER NOTING 'GRAHM 10000 FT' ON THE DSCNT PAGE AS I SCROLLED PAST IT. WE WERE SUBSEQUENTLY VECTORED FOR TFC TWICE AND GIVEN A COUPLE OF ALT CHANGES, ALL OF WHICH I ENTERED INTO THE FMS. FINALLY, THE CTLR ISSUED THE XING OF GRAHM AT 10000 FT. I DSNDED USING THE FMS, INSTEAD OF USING MY HEAD! AT APPROX 15-20 MI FROM GRAHM INTXN THE CTLR ASKED IF WE WERE GOING TO MAKE THE RESTR. I IMMEDIATELY LOOKED DOWN ON THE DSCNT PAGE AND NOTICED 'GRAHM, 10000 FT' WAS NO LONGER THERE. WE ASKED FOR AND RECEIVED A TURN FROM ATC, BECAUSE WE WERE NOT GOING TO MAKE THE RESTR. WE APOLOGIZED TO THE CTLR AND ASKED IF THERE WERE ANY CONFLICTS. HE ADVISED US THERE WERE NONE, AND THE REST OF THE FLT WAS UNEVENTFUL. THIS WAS THE LAST FLYING LEG OF MY TRIP, BEFORE DEADHEADING BACK TO BASE. ALTHOUGH THIS WAS THE FIFTH CONSECUTIVE DAY WORKING, I WAS NOT FATIGUED. THIS EVENT SHOULD HAVE BEEN PREVENTED, IF ONLY I WOULD HAVE DONE WHAT I MOST CERTAINLY ALWAYS DO. WHEN GIVEN A XING RESTR, I IMMEDIATELY GO TO THE LEGS PAGE OF THE FMS TO ENSURE THE XING IS ENTERED. MORE IMPORTANTLY, I ALWAYS BACK UP MYSELF AND THE FMS BY DOING THE 'MENTAL MATH' DURING THE DSCNT. IN THIS CASE, HOWEVER, I DID NEITHER AND RELIED COMPLETELY ON THE FMS INSTEAD OF USING MY HEAD. UNFORTUNATELY, I AM STILL AT A LOSS AS TO WHY I DIDN'T.
