Narrative:

Late at night left chicago, palwaukee airport XA00 pm CST or XB00 am EST. Uneventful flight to hzy. Set up GPS runway 8 approach. Talking to cleveland. Cleveland advised to change to local frequency. At that point I thought I had both cleveland and local dialed into my 2 radios as I tuned in hzy 122.8 on communication #2 and left cleveland on communication #1 (don't remember the frequency). When I idented the runway at approximately 800 ft AGL (1725 ft), I announced cancellation of the flight plan and proceeded to land. I, thinking about the event, don't recall cleveland acknowledging cancellation, which, in hindsight, should have prompted me to call again from the air or call FSS on the ground. Instead, I received a call at XF15 am at home from either FSS or some other agency (my wife said it was the FAA. She answered the phone). I explained I was sure I had canceled, but perhaps the cancellation was not received. Lesson learned: I was not tired so that is no excuse. I got plenty of sleep the night before the flight to and from chicago in anticipation of the need. The primary reason was to confirm complete communication with ATC or other advisory services in order to ensure understanding of the pilot's intentions.

Title: A PA24 PLT FORGETS TO CANCEL IFR CLRNC.

Narrative: LATE AT NIGHT LEFT CHICAGO, PALWAUKEE ARPT XA00 PM CST OR XB00 AM EST. UNEVENTFUL FLT TO HZY. SET UP GPS RWY 8 APCH. TALKING TO CLEVELAND. CLEVELAND ADVISED TO CHANGE TO LCL FREQ. AT THAT POINT I THOUGHT I HAD BOTH CLEVELAND AND LCL DIALED INTO MY 2 RADIOS AS I TUNED IN HZY 122.8 ON COM #2 AND LEFT CLEVELAND ON COM #1 (DON'T REMEMBER THE FREQ). WHEN I IDENTED THE RWY AT APPROX 800 FT AGL (1725 FT), I ANNOUNCED CANCELLATION OF THE FLT PLAN AND PROCEEDED TO LAND. I, THINKING ABOUT THE EVENT, DON'T RECALL CLEVELAND ACKNOWLEDGING CANCELLATION, WHICH, IN HINDSIGHT, SHOULD HAVE PROMPTED ME TO CALL AGAIN FROM THE AIR OR CALL FSS ON THE GND. INSTEAD, I RECEIVED A CALL AT XF15 AM AT HOME FROM EITHER FSS OR SOME OTHER AGENCY (MY WIFE SAID IT WAS THE FAA. SHE ANSWERED THE PHONE).
