Narrative:
Halfway through our flight, the flight attendants asked me how long before landing. They said that the 2 therapeutic oxygen bottles that one of our passenger was using would just be enough to get us to new york, and to please advise them of any en route delays. I mentioned that I usually receive a form X for such oxygen bottles, and that I was unaware we were carrying them. These are the new oxygen bottles that are carried in a green canvas bag. The flight attendants searched the oxygen bottles and found a form X for 'dangerous goods' hidden in one of the bag's compartments. This form listed 3 oxygen bottles, but the flight attendants confirmed that only 2 oxygen bottles were brought on board. In addition, the only signature was in the prepared by block. The other 2 blocks were blank. Part 1 'XXX' Z says that all oxygen bottles must be accompanied by a form X. I guess that passenger service took this literally. By putting the form inside the pouch of the bottle, the form would definitely accompany the oxygen bottles. In fact, part 1 implies that each oxygen bottle should have a separate form X. Although only 2 bottles were needed, a third bottle would have helped in case of any en route delays, or diversions.

Title: A B767 WAS DISPATCHED IN NON COMPLIANCE WITH A PAX USING THERAPEUTIC OXYGEN WITH NO HAZMAT FORM GIVEN TO THE FLC.
Narrative: HALFWAY THROUGH OUR FLT, THE FLT ATTENDANTS ASKED ME HOW LONG BEFORE LNDG. THEY SAID THAT THE 2 THERAPEUTIC OXYGEN BOTTLES THAT ONE OF OUR PAX WAS USING WOULD JUST BE ENOUGH TO GET US TO NEW YORK, AND TO PLEASE ADVISE THEM OF ANY ENRTE DELAYS. I MENTIONED THAT I USUALLY RECEIVE A FORM X FOR SUCH OXYGEN BOTTLES, AND THAT I WAS UNAWARE WE WERE CARRYING THEM. THESE ARE THE NEW OXYGEN BOTTLES THAT ARE CARRIED IN A GREEN CANVAS BAG. THE FLT ATTENDANTS SEARCHED THE OXYGEN BOTTLES AND FOUND A FORM X FOR 'DANGEROUS GOODS' HIDDEN IN ONE OF THE BAG'S COMPARTMENTS. BY PUTTING THE FORM INSIDE THE POUCH OF THE BOTTLE, THE FORM WOULD DEFINITELY ACCOMPANY THE OXYGEN BOTTLES. IN FACT, PART 1 IMPLIES THAT EACH OXYGEN BOTTLE SHOULD HAVE A SEPARATE FORM X. ALTHOUGH ONLY 2 BOTTLES WERE NEEDED, A THIRD BOTTLE WOULD HAVE HELPED IN CASE OF ANY ENRTE DELAYS, OR DIVERSIONS.
