Narrative:
On the flight airline XXX from sfo to phl, a preflight security form was not completed. The form was not present in the aircraft galley, jetway or handed to me by the agent. No one from the ramp, baggage, catering or any other person presented it, or in any way was the form mentioned. It wasn't until we had almost arrived at our destination of phl that it occurred to me that the form wasn't completed. I am assuming it was an originating flight although I am not certain if that was the case. I contacted the 'a' flight attendant and asked her if she saw the form or if anyone had mentioned it and she said no. She also was not sure when the form is even required. Thinking about it, the things that trigger me into even thinking about this form are seeing it in the galley or someone handing it to me. It is usually after others have completed their portions that I become involved just prior to passenger boarding. I've noted the following -- there is no cockpit checklist item specifically referencing the form as being completed. There is no designated place for this form. I've seen it on the jetway, on the galley shelf, hanging up, on the floor, or given to me by the agent, but it's never consistent. The agents do not specifically mention it in their predep cockpit checks, as they do with the passenger count, fuel load or crew member verification. The flight attendants are not in the loop as they are not even aware of when it's required. There is no longer any mention of it on the flight release. I believe that proper procedures as to the verification and implementation of this form are as important as the security checks themselves. I believe that the lack of sound procedures contributed to this event and hope that these suggestions will help correct them and prevent future omissions.

Title: AN A321 CREW, DEPARTING SFO, NEGLECTED TO SIGN THE SECURITY FORM.
Narrative: ON THE FLT AIRLINE XXX FROM SFO TO PHL, A PREFLT SECURITY FORM WAS NOT COMPLETED. THE FORM WAS NOT PRESENT IN THE ACFT GALLEY, JETWAY OR HANDED TO ME BY THE AGENT. I'VE SEEN IT ON THE JETWAY, ON THE GALLEY SHELF, HANGING UP, ON THE FLOOR, OR GIVEN TO ME BY THE AGENT, BUT IT'S NEVER CONSISTENT. THE AGENTS DO NOT SPECIFICALLY MENTION IT IN THEIR PREDEP COCKPIT CHKS, AS THEY DO WITH THE PAX COUNT, FUEL LOAD OR CREW MEMBER VERIFICATION. I BELIEVE THAT THE LACK OF SOUND PROCS CONTRIBUTED TO THIS EVENT AND HOPE THAT THESE SUGGESTIONS WILL HELP CORRECT THEM AND PREVENT FUTURE OMISSIONS.
