Narrative:
I'm a new instructor pilot teaching advanced jet training for the navy. I have 4000+ hours in tactical carrier-based jets (F18/F5). All single plted aircraft. I have just begun teaching students and flying in a multi-pilot situation. The point of this safety report is 1) crew coordination, and 2) communication responsibilities. Everyone involved with air transportation must be a safety officer. It is the responsibility of all to work together to prevent mishaps. All must be vigilant in the identify of unsafe practices, complacency, factors which could begin a critical chain of events that might lead to an unfortunate conclusion. No one person is above procedure, or beyond reproach. We all make mistakes, so we must all rely on each other to continually 'check our six' to ensure that safety is paramount. Human factors will never be eliminated completely as long as the 'human' machine is in the loop. We must all strive for continued education, learn from our mistakes, be open and honest and accept criticism, and above all work together to continue to improve on a daily basis.
