Narrative:

135 check ride. I started the aircraft at XA20 am, taxied to the end of runway and completed the run-up. We were cleared into position to hold at XA30 am. After a short time we were cleared for takeoff. I applied both throttles and I started the takeoff. The check pilot failed the left engine. I failed to retard the right throttle quickly enough, and we exited the left side of the runway approximately 300 ft from the hold position. We came to a stop 10-12 ft off the side of the runway. As for a corrective action the practice of engine failure procedures on takeoff roll is essential.

Title: A C421 PLT, DEPARTING OGD ON A CHK RIDE, DEPARTED THE L SIDE OF THE RWY WHEN CHK PLT FAILED AN ENG SHORTLY AFTER THROTTLES WERE ADVANCED FOR TKOF.

WE CAME TO A STOP 10-12 FT OFF THE SIDE OF THE RWY. AS FOR A CORRECTIVE ACTION THE PRACTICE OF ENG FAILURE PROCS ON TKOF ROLL IS ESSENTIAL.
