Narrative:

I was captain/PNF on a hawaii to lax flight. We had FMC/IAS problems which caused us to depart 1 hour 35 mins late. Maintenance blamed me for the FMC problems. I have 7000+ hours on this glass and know how to program fuel and winds. The local mechanic finally saw the FMC dump on its own and got us off the hook for causing the FMC problem. We also had a fuel problem where the fueler couldn't put on enough fuel for dispatch so we had to call dispatch to lower the fuel. The dispatcher did not understand me so it took some time to lower the fuel for dispatch. During that time we had to brief 3 runway changes due to heavy rain/wind and maximum runway weight limits. We loaded the FMC 5 times at 12 mins per load. We had to perform glass ii navigation checks 5 times. It was night in driving rain, departure time scheduled was XA25 body time/XC01 body time actual. We started engines and started taxiing. My first officer selected flaps 5 degrees not flaps 20 degrees as briefed and set up. He read the checklist and I responded to flaps by saying 5 degrees planned, 5 degrees indicated detent. My major error and my responsibility, my mistake. As we took the runway I made my end of runway check of flaps, spoilers and trim. I still missed flaps 20 degrees. I did have a bad feeling though. As I prepared for the 80 KTS check I scanned the power tree and saw flaps 5 degrees and yelled flaps 20 degrees and told my first officer to continue. I immediately selected flaps 20 degrees and made my 80 KT call between 100 and 110 KTS. The flaps made flaps 20 degrees position prior to V1. My first officer delayed rotation till V2. All other takeoff procedures were non eventful. On arrival to lax, I self-disclosed to the fodm and pilots organization and fleet and my flight office. Had I not caught the problem we would have probably had a tail strike or run off the end. I believe in integrity and honesty the company would have never known if I had not come forward and reported. I hope they don't beat me up too much. My direct boss was great, I don't know about anyone else. Factors: night backside of clock. Actual departure 1 hour 35 mins late. Early am body time. Driving rain. Runway limit weight. Routine of flaps 5 degrees. 4 hours daytime sleep prior to event. Short runway. 3RD trip to lih in 2 yrs. Maintenance blaming me for not programming FMC correctly. Local mechanic telling me 'I'm going to get it in ca for the delay.' my own time pressure to get going. Worry over disciplinary action over FMC problem. I forgot flaps 20 degrees on taxi out. End of runway check didn't see problem. Fixed problem by V1.

Title: EXTENDED TKOF ROLL AS PIC NOTES IMPROPERLY SET WING FLAPS AT 5 DEGS VERSUS 20 DEGS FOR A MAX PERFORMANCE NIGHT OP TKOF IN INCLEMENT CONDITIONS AT LIH, HI.

Narrative: I WAS CAPT/PNF ON A HAWAII TO LAX FLT. WE HAD FMC/IAS PROBS WHICH CAUSED US TO DEPART 1 HR 35 MINS LATE. MAINT BLAMED ME FOR THE FMC PROBS. I HAVE 7000+ HRS ON THIS GLASS AND KNOW HOW TO PROGRAM FUEL AND WINDS. THE LCL MECH FINALLY SAW THE FMC DUMP ON ITS OWN AND GOT US OFF THE HOOK FOR CAUSING THE FMC PROB. WE ALSO HAD A FUEL PROB WHERE THE FUELER COULDN'T PUT ON ENOUGH FUEL FOR DISPATCH SO WE HAD TO CALL DISPATCH TO LOWER THE FUEL. THE DISPATCHER DID NOT UNDERSTAND ME SO IT TOOK SOME TIME TO LOWER THE FUEL FOR DISPATCH. DURING THAT TIME WE HAD TO BRIEF 3 RWY CHANGES DUE TO HVY RAIN/WIND AND MAX RWY WT LIMITS. WE LOADED THE FMC 5 TIMES AT 12 MINS PER LOAD. WE HAD TO PERFORM GLASS II NAV CHKS 5 TIMES. IT WAS NIGHT IN DRIVING RAIN, DEP TIME SCHEDULED WAS XA25 BODY TIME/XC01 BODY TIME ACTUAL. WE STARTED ENGS AND STARTED TAXIING. MY FO SELECTED FLAPS 5 DEGS NOT FLAPS 20 DEGS AS BRIEFED AND SET UP. HE READ THE CHKLIST AND I RESPONDED TO FLAPS BY SAYING 5 DEGS PLANNED, 5 DEGS INDICATED DETENT. MY MAJOR ERROR AND MY RESPONSIBILITY, MY MISTAKE. AS WE TOOK THE RWY I MADE MY END OF RWY CHK OF FLAPS, SPOILERS AND TRIM. I STILL MISSED FLAPS 20 DEGS. I DID HAVE A BAD FEELING THOUGH. AS I PREPARED FOR THE 80 KTS CHK I SCANNED THE PWR TREE AND SAW FLAPS 5 DEGS AND YELLED FLAPS 20 DEGS AND TOLD MY FO TO CONTINUE. I IMMEDIATELY SELECTED FLAPS 20 DEGS AND MADE MY 80 KT CALL BTWN 100 AND 110 KTS. THE FLAPS MADE FLAPS 20 DEGS POS PRIOR TO V1. MY FO DELAYED ROTATION TILL V2. ALL OTHER TKOF PROCS WERE NON EVENTFUL. ON ARR TO LAX, I SELF-DISCLOSED TO THE FODM AND PLTS ORGANIZATION AND FLEET AND MY FLT OFFICE. HAD I NOT CAUGHT THE PROB WE WOULD HAVE PROBABLY HAD A TAIL STRIKE OR RUN OFF THE END. I BELIEVE IN INTEGRITY AND HONESTY THE COMPANY WOULD HAVE NEVER KNOWN IF I HAD NOT COME FORWARD AND RPTED. I HOPE THEY DON'T BEAT ME UP TOO MUCH. MY DIRECT BOSS WAS GREAT, I DON'T KNOW ABOUT ANYONE ELSE. FACTORS: NIGHT BACKSIDE OF CLOCK. ACTUAL DEP 1 HR 35 MINS LATE. EARLY AM BODY TIME. DRIVING RAIN. RWY LIMIT WT. ROUTINE OF FLAPS 5 DEGS. 4 HRS DAYTIME SLEEP PRIOR TO EVENT. SHORT RWY. 3RD TRIP TO LIH IN 2 YRS. MAINT BLAMING ME FOR NOT PROGRAMMING FMC CORRECTLY. LCL MECH TELLING ME 'I'M GOING TO GET IT IN CA FOR THE DELAY.' MY OWN TIME PRESSURE TO GET GOING. WORRY OVER DISCIPLINARY ACTION OVER FMC PROB. I FORGOT FLAPS 20 DEGS ON TAXI OUT. END OF RWY CHK DIDN'T SEE PROB. FIXED PROB BY V1.
