Narrative:
I served as first officer on flight mia-ccs-bog. After landing in bog, flight engineer discussed with captain about fuel used counter inoperative along with a fuel flow indicator already deferred. I was not included in discussion about this maintenance discrepancy, but do recall them looking in the deferred MEL book and agreeing it was ok. I subsequently took the next flight bog-mia with a different captain who did not question the deference of both fuel flow and used counter. Reassigned once again to next flight on same aircraft mia-ccs-bog with a different captain. He researched MEL and found it to be unapproved for which I read MEL and concur. Maintenance replaced part, item operations normal, dmi cleared. It is not normal policy/procedure to include first officer into discussion, especially after landing, postflt write-ups. There is also a page within our MEL that allows this combination, but for a pratt and whitney engine for which this company does not operate. This occurrence slipped through several layers of protection, from flight crew, line maintenance, and maintenance control.

Title: NON COMPLIANCE WITH MEL REQUIREMENTS ON A B747-300 CARGO ACFT WHEN FUEL USED COUNTER IS ADDED TO DEFERRED LIST ALONG WITH AN INOP ENG FUEL FLOW INDICATOR AT SKBO, FO.
Narrative: I SERVED AS FO ON FLT MIA-CCS-BOG. AFTER LNDG IN BOG, FE DISCUSSED WITH CAPT ABOUT FUEL USED COUNTER INOP ALONG WITH A FUEL FLOW INDICATOR ALREADY DEFERRED. I WAS NOT INCLUDED IN DISCUSSION ABOUT THIS MAINT DISCREPANCY, BUT DO RECALL THEM LOOKING IN THE DEFERRED MEL BOOK AND AGREEING IT WAS OK. I SUBSEQUENTLY TOOK THE NEXT FLT BOG-MIA WITH A DIFFERENT CAPT WHO DID NOT QUESTION THE DEFERENCE OF BOTH FUEL FLOW AND USED COUNTER. REASSIGNED ONCE AGAIN TO NEXT FLT ON SAME ACFT MIA-CCS-BOG WITH A DIFFERENT CAPT. HE RESEARCHED MEL AND FOUND IT TO BE UNAPPROVED FOR WHICH I READ MEL AND CONCUR. MAINT REPLACED PART, ITEM OPS NORMAL, DMI CLRED. IT IS NOT NORMAL POLICY/PROC TO INCLUDE FO INTO DISCUSSION, ESPECIALLY AFTER LNDG, POSTFLT WRITE-UPS. THERE IS ALSO A PAGE WITHIN OUR MEL THAT ALLOWS THIS COMBINATION, BUT FOR A PRATT AND WHITNEY ENG FOR WHICH THIS COMPANY DOES NOT OPERATE. THIS OCCURRENCE SLIPPED THROUGH SEVERAL LAYERS OF PROTECTION, FROM FLC, LINE MAINT, AND MAINT CTL.
