Narrative:
Maintenance installed wrong girt bar aft door. Flight attendants called 1 hour into flight. Flight attendant #2 and flight attendant #1 did not really check L2 girt bar locked in place. Maintenance installed wrong size girt bar on slide pack change. Flight attendants thought girt bar was locked, sounded locked but was not. Girt bar stayed in bracket which made bar look locked. Pressure from slide pack was enough to hold bar into bracket. Recommended improvements: maintenance quality assurance has to actually check girt bar locked -- don't change girt bars on slide pack change. Flight attendants have to be hands on check and xchk of girt bars. Last min rush before push is a real zoo!

Title: B737 CREW WAS INFORMED BY THE FLT ATTENDANT THAT THE EMER ESCAPE SLIDE GIRT BAR WAS NOT ATTACHED TO THE DOOR FRAME PROPERLY. THEY WERE INFORMED AFTER THE ACFT WAS AIRBORNE.
Narrative: MAINT INSTALLED WRONG GIRT BAR AFT DOOR. FLT ATTENDANTS CALLED 1 HR INTO FLT. FLT ATTENDANT #2 AND FLT ATTENDANT #1 DID NOT REALLY CHK L2 GIRT BAR LOCKED IN PLACE. MAINT INSTALLED WRONG SIZE GIRT BAR ON SLIDE PACK CHANGE. FLT ATTENDANTS THOUGHT GIRT BAR WAS LOCKED, SOUNDED LOCKED BUT WAS NOT. GIRT BAR STAYED IN BRACKET WHICH MADE BAR LOOK LOCKED. PRESSURE FROM SLIDE PACK WAS ENOUGH TO HOLD BAR INTO BRACKET. RECOMMENDED IMPROVEMENTS: MAINT QUALITY ASSURANCE HAS TO ACTUALLY CHK GIRT BAR LOCKED -- DON'T CHANGE GIRT BARS ON SLIDE PACK CHANGE. FLT ATTENDANTS HAVE TO BE HANDS ON CHK AND XCHK OF GIRT BARS. LAST MIN RUSH BEFORE PUSH IS A REAL ZOO!
