Narrative:
I have had 2 failures of the #3 brake system in the last 2-3 months. The first was in mia. It was a complete failure of the #3 brake assembly. When maintenance removed the brake assembly what was left of the brake pads fell to the ground along with other metal. The rotors were deformed from the heat and loose pieces which fell to the ground. The second was in sna on oct/sun/02. The rivets holding the inner most pad appeared to be sheared. The pad failed and the inner most lining was crumpled from the loose pieces and deformed. In both instances there was a difference in the brake wear indicator pins front versus rear. I would also suggest a bulletin alerting flight and maintenance personnel to pay particular attention to the brake wear indicator pins. A difference in the wear of the front versus the rear pin would indicate a probable failure of the associated brake assembly. Maintenance should remove and inspect all assemblies with a wear difference indicated by the pins.

Title: A B737-800 CAPT RPTS 2 CATASTROPHIC #3 BRAKE ASSEMBLY FAILURES IN A 60-90 DAY PERIOD. NOTED DIFFERENCE BTWN FRONT AND REAR WEAR PINS PRIOR TO FAILURE.
Narrative: I HAVE HAD 2 FAILURES OF THE #3 BRAKE SYS IN THE LAST 2-3 MONTHS. THE FIRST WAS IN MIA. IT WAS A COMPLETE FAILURE OF THE #3 BRAKE ASSEMBLY. WHEN MAINT REMOVED THE BRAKE ASSEMBLY WHAT WAS LEFT OF THE BRAKE PADS FELL TO THE GND ALONG WITH OTHER METAL. THE ROTORS WERE DEFORMED FROM THE HEAT AND LOOSE PIECES WHICH FELL TO THE GND. THE SECOND WAS IN SNA ON OCT/SUN/02. THE RIVETS HOLDING THE INNER MOST PAD APPEARED TO BE SHEARED. THE PAD FAILED AND THE INNER MOST LINING WAS CRUMPLED FROM THE LOOSE PIECES AND DEFORMED. IN BOTH INSTANCES THERE WAS A DIFFERENCE IN THE BRAKE WEAR INDICATOR PINS FRONT VERSUS REAR. I WOULD ALSO SUGGEST A BULLETIN ALERTING FLT AND MAINT PERSONNEL TO PAY PARTICULAR ATTN TO THE BRAKE WEAR INDICATOR PINS. A DIFFERENCE IN THE WEAR OF THE FRONT VERSUS THE REAR PIN WOULD INDICATE A PROBABLE FAILURE OF THE ASSOCIATED BRAKE ASSEMBLY. MAINT SHOULD REMOVE AND INSPECT ALL ASSEMBLIES WITH A WEAR DIFFERENCE INDICATED BY THE PINS.
