Narrative:

After arriving at msp from a las red eye, we were distraction during shutdown. The flight attendant from door #4 called after first engine shutdown (concerning disarming the door). At this point we were still waiting for external power so we could shut down the second engine. We proceeded to fill out logbooks and secure the cockpit. We were not at any time in a hurry, but we were fatigued. We left feeling we had done our jobs properly. We were later notified that the left fuel cutoff switch had not been cut off and the parking flow not complete. Maintenance says they tried to contact us, but we apparently didn't hear. I have no memory of forgetting this flow or checklist. The only contributing factor I can think of is fatigue. It was the last leg of a minimum rest 5 day trip. It was an unfortunate mistake caused by a distraction and no harm was done to the aircraft or any person.

Title: FLC FAILS TO SHUT OFF ENG AT ARR GATE.

Narrative: AFTER ARRIVING AT MSP FROM A LAS RED EYE, WE WERE DISTR DURING SHUTDOWN. THE FLT ATTENDANT FROM DOOR #4 CALLED AFTER FIRST ENG SHUTDOWN (CONCERNING DISARMING THE DOOR). AT THIS POINT WE WERE STILL WAITING FOR EXTERNAL PWR SO WE COULD SHUT DOWN THE SECOND ENG. WE PROCEEDED TO FILL OUT LOGBOOKS AND SECURE THE COCKPIT. WE WERE NOT AT ANY TIME IN A HURRY, BUT WE WERE FATIGUED. WE LEFT FEELING WE HAD DONE OUR JOBS PROPERLY. WE WERE LATER NOTIFIED THAT THE L FUEL CUTOFF SWITCH HAD NOT BEEN CUT OFF AND THE PARKING FLOW NOT COMPLETE. MAINT SAYS THEY TRIED TO CONTACT US, BUT WE APPARENTLY DIDN'T HEAR. I HAVE NO MEMORY OF FORGETTING THIS FLOW OR CHKLIST. THE ONLY CONTRIBUTING FACTOR I CAN THINK OF IS FATIGUE. IT WAS THE LAST LEG OF A MINIMUM REST 5 DAY TRIP. IT WAS AN UNFORTUNATE MISTAKE CAUSED BY A DISTR AND NO HARM WAS DONE TO THE ACFT OR ANY PERSON.
