Narrative:
Course deviation inbound to las on the skebr arrival. I, first officer, selected the ILS runway 25L, then hit legs page to close the discontinuity up. When I did this 'shand' was on the legs page. Knowing we would not go there, I selected the GS runway 25L and closed the 'discontinuity,' not realizing that the 'kepec, ipumy and povoc' were not in the box. I discovered it the same time the controller did and turned left to a heading of 340 degrees. Factors -- captain loaded the box, and I did not check the transition, and when selecting runway 25L, the box doesn't prompt the transition. When loading the box with ONTLAS01, it will load to skebr. Prevention: always doublechk and verify what has been entered into the box. When selecting a runway, make sure it includes a transition or whatever appropriate fixes are on the arrival. Now that we select the runway 25L transition into las, it seems the box should load the appropriate fixes. Once again, it all has to be verified by 'both' pilots. Supplemental information from acn 572578: what I noted: we had arrival and runway 25L loaded. It looked correct at both ends, but I failed to note kepec, ipumy, povoc, and kimme intxns were missing. Interesting notes: I loaded the FMC again to try to find my error and found 3 rtes. 1) load company route, then add runway 25L, we get the missing intxns. 2) load arrival and runway 25L, and we get the wrong intxns (shand) and no kepec. 3) if you load arrival, runway 25L and dag transition, it will load correctly. Conclusion: verify, verify, verify. I watched the first officer input the correct data, but the output was not as expected.
Original NASA ASRS Text
Title: A COURSE DEV ON THE SKEBER RNAV 2 ARR TO LAS, NEV. QUICKLY CORRECTED BY ATC.
Narrative: COURSE DEV INBOUND TO LAS ON THE SKEBR ARR. I, FO, SELECTED THE ILS RWY 25L, THEN HIT LEGS PAGE TO CLOSE THE DISCONTINUITY UP. WHEN I DID THIS 'SHAND' WAS ON THE LEGS PAGE. KNOWING WE WOULD NOT GO THERE, I SELECTED THE GS RWY 25L AND CLOSED THE 'DISCONTINUITY,' NOT REALIZING THAT THE 'KEPEC, IPUMY AND POVOC' WERE NOT IN THE BOX. I DISCOVERED IT THE SAME TIME THE CTLR DID AND TURNED L TO A HDG OF 340 DEGS. FACTORS -- CAPT LOADED THE BOX, AND I DID NOT CHK THE TRANSITION, AND WHEN SELECTING RWY 25L, THE BOX DOESN'T PROMPT THE TRANSITION. WHEN LOADING THE BOX WITH ONTLAS01, IT WILL LOAD TO SKEBR. PREVENTION: ALWAYS DOUBLECHK AND VERIFY WHAT HAS BEEN ENTERED INTO THE BOX. WHEN SELECTING A RWY, MAKE SURE IT INCLUDES A TRANSITION OR WHATEVER APPROPRIATE FIXES ARE ON THE ARR. NOW THAT WE SELECT THE RWY 25L TRANSITION INTO LAS, IT SEEMS THE BOX SHOULD LOAD THE APPROPRIATE FIXES. ONCE AGAIN, IT ALL HAS TO BE VERIFIED BY 'BOTH' PLTS. SUPPLEMENTAL INFO FROM ACN 572578: WHAT I NOTED: WE HAD ARR AND RWY 25L LOADED. IT LOOKED CORRECT AT BOTH ENDS, BUT I FAILED TO NOTE KEPEC, IPUMY, POVOC, AND KIMME INTXNS WERE MISSING. INTERESTING NOTES: I LOADED THE FMC AGAIN TO TRY TO FIND MY ERROR AND FOUND 3 RTES. 1) LOAD COMPANY RTE, THEN ADD RWY 25L, WE GET THE MISSING INTXNS. 2) LOAD ARR AND RWY 25L, AND WE GET THE WRONG INTXNS (SHAND) AND NO KEPEC. 3) IF YOU LOAD ARR, RWY 25L AND DAG TRANSITION, IT WILL LOAD CORRECTLY. CONCLUSION: VERIFY, VERIFY, VERIFY. I WATCHED THE FO INPUT THE CORRECT DATA, BUT THE OUTPUT WAS NOT AS EXPECTED.
