Narrative:
Aircraft mechanic/refueling crew misfueled aircraft. This resulted in 6000 pounds left wing heavy condition. Mechanic/refueling crew did not understand fuel gauge inoperative procedures (right inner tank fuel gauge was inoperative). Aircraft maintenance log entry was incorrect and showed the wrong fuel on the aircraft.
