Narrative:

I experienced a nose gear collapse upon takeoff at norwood airport. On approach to norwood, I experienced landing gear light indications that the landing gear was in the down and locked position. Upon touchdown at owd, the landing gear transition light came on. Landing and taxi were normal. After confirming with my home base via telephone, it was decided that if a mechanic could verify my landing gear was in the down and locked position, a safe return to my home base was reasonable. I had a mechanic at owd check the landing gear and was assured by him that the landing gear was down and locked. The following run-up and taxi were normal. Upon takeoff, the nosewheel collapsed resulting in a 'propeller strike.' a go around was initiated, gear was recycled, emergency was declared and I landed plane. There was no injury to pilot or passenger, and no additional frame damage.

Title: SMA PLT HAD THE NOSE GEAR COLLAPSE ON TKOF. THE PLT HAD NOSE GEAR PROBS ON LNDG. LCL MAINT APPROVED OP OF THE ACFT IN THIS CONDITION.

Narrative: I EXPERIENCED A NOSE GEAR COLLAPSE UPON TKOF AT NORWOOD ARPT. ON APCH TO NORWOOD, I EXPERIENCED LNDG GEAR LIGHT INDICATIONS THAT THE LNDG GEAR WAS IN THE DOWN AND LOCKED POS. UPON TOUCHDOWN AT OWD, THE LNDG GEAR TRANSITION LIGHT CAME ON. LNDG AND TAXI WERE NORMAL. AFTER CONFIRMING WITH MY HOME BASE VIA TELEPHONE, IT WAS DECIDED THAT IF A MECH COULD VERIFY MY LNDG GEAR WAS IN THE DOWN AND LOCKED POS, A SAFE RETURN TO MY HOME BASE WAS REASONABLE. I HAD A MECH AT OWD CHK THE LNDG GEAR AND WAS ASSURED BY HIM THAT THE LNDG GEAR WAS DOWN AND LOCKED. THE FOLLOWING RUN-UP AND TAXI WERE NORMAL. UPON TKOF, THE NOSEWHEEL COLLAPSED RESULTING IN A 'PROP STRIKE.' A GAR WAS INITIATED, GEAR WAS RECYCLED, EMER WAS DECLARED AND I LANDED PLANE. THERE WAS NO INJURY TO PLT OR PAX, AND NO ADDITIONAL FRAME DAMAGE.
