Narrative:

Flight from den-dik we had to return to den after 'smoke' (master warning). It was the result of a failure of the right pack. At leveloff (FL290) we turned the packs from low to norm. We immediately noted a T6 spike on the right engine of +35 degrees C as compared to +3 degrees on the left engine. We reduced power to compensate for the rise. Within seconds we smelled a hot smell. Because it was immediate and the xbleed was closed we turned the left pack back to the low setting. Within seconds we got the 'smoke' and we procceded with the memory items for 'smoke.' after we both were ready we proceeded with eap. When we got to the note about the 13K switch the decision was made to turn off the oxygen masks, because the 'smoke' extinguished and pressurization was under control. The guarded switch was improperly installed, and resulted in mask deployment. The decision was made to return to den.

Title: EMB120 FLT CREW HAS SMOKE AND FUMES DURING CLBOUT FROM DEN.

Narrative: FLT FROM DEN-DIK WE HAD TO RETURN TO DEN AFTER 'SMOKE' (MASTER WARNING). IT WAS THE RESULT OF A FAILURE OF THE R PACK. AT LEVELOFF (FL290) WE TURNED THE PACKS FROM LOW TO NORM. WE IMMEDIATELY NOTED A T6 SPIKE ON THE R ENG OF +35 DEGS C AS COMPARED TO +3 DEGS ON THE L ENG. WE REDUCED PWR TO COMPENSATE FOR THE RISE. WITHIN SECONDS WE SMELLED A HOT SMELL. BECAUSE IT WAS IMMEDIATE AND THE XBLEED WAS CLOSED WE TURNED THE L PACK BACK TO THE LOW SETTING. WITHIN SECONDS WE GOT THE 'SMOKE' AND WE PROCCEDED WITH THE MEMORY ITEMS FOR 'SMOKE.' AFTER WE BOTH WERE READY WE PROCEEDED WITH EAP. WHEN WE GOT TO THE NOTE ABOUT THE 13K SWITCH THE DECISION WAS MADE TO TURN OFF THE OXYGEN MASKS, BECAUSE THE 'SMOKE' EXTINGUISHED AND PRESSURIZATION WAS UNDER CTL. THE GUARDED SWITCH WAS IMPROPERLY INSTALLED, AND RESULTED IN MASK DEPLOYMENT. THE DECISION WAS MADE TO RETURN TO DEN.
