Narrative:

During pushback a combination of hand signals and headset voice led to some hand signals being missed. The hand signals to set the brake was not confirmed or received by the flight crew. The aircraft was detached from the tug with the parking brake off. The aircraft started to roll and was stopped. A corrective action would be if hand signals and headsets are used at the same time all phraseology should still be spoken.
