Narrative:

Departure salute. No member of the push crew was visible from the cockpit from the time they were cleared to disconnect interphone until the time they rode out from under the aircraft nose on the tug. It is normal procedure to have a guide man visible with crossed wands until the area is clear for the aircraft to taxi. This is my third debrief on this problem. So far, nothing has changed.

Title: B757-200 CAPT NOTED THE CREWS DISPATCHING THE ACFT FROM THE GATE, ARE NOT FOLLOWING ACCEPTED COMPANY PROCS. THIS REQUIRES A GUIDE MAN WITH WANDS, TO STAND WITH WANDS CROSSED, IN FULL VIEW OF THE CAPT, UNTIL THE ACFT IS CLR TO MOVE.
