Narrative:

I have been flying north atlantic trips where we have 3 man cockpit and 3 man xchks. This is my first 2 man crew in awhile. It is possible something was missed. This is not an attempt to make any excuses but to try and resolve this issue. After finishing my flight planning in bda operations, I went to the aircraft. The first officer was in the cockpit and I asked him if all was well with the preflight and the aircraft. He said yes. He also said that he was having trouble up-loading the flight plan to the FMC from the CDU. I manually loaded the flight plan in the FMC and then we both checked the route, legs and the mileage. All checked okay. I thought at that time I checked the navigation data like I always do as required. I feel sure I did. After pushing and starting, we taxied to runway 12. This is a very short taxi. After completing taxi checklist, we were given clearance to take the runway and takeoff. The aircraft headings checked and all looked normal. We were looking directly into the sun at this time. Both of us were trying to adjust and shade our eyes somewhat from the direct sunlight. The first officer was flying. During takeoff, we got one aural pull up from the GPWS system. At 3000 ft, ZNY gave us a left turn direct gabes and a climb to 25000 ft. At 23000 ft and inside of 30 mi from bda, ZNY asked my heading. It was 330 degrees. The controller said that we need to turn to 310 degrees. I asked the controller to plot our original heading. He said it would put us 60-80 mi off course at gabes. I immediately looked at all 3 IRU's. All 3 had similar data. I asked if we could orbit until I looked at everything. I checked the raw data with the VOR on bda and the fix page on the CDU. I could still see bda. What I could see on the raw data and what I saw looking from the aircraft, was different. It was obvious that we had a navigation failure. I contacted bda operations and asked them to advise dispatch and maintenance. I then elected to return to bda and deal with the problem on the ground. During the approach and landing, the aircraft continued to sound 'pull-up' until after we cleared the runway and retracted the flaps. The aircraft had no idea where it was. After we taxied to the gate, I asked the first officer how he entered the navigation data to align the IRU's. He said that he used normal procedures. He then said he went outside to do his walkaround. After returning inside, he said that he noticed that the align lights were flashing. He said that he reentered the navigation data into the IRU's and that all went well. I asked him how many times he put the data in. He said twice. I did not know any of this had taken place until we returned to the gate in bda. Supplemental information from acn 599594: when I returned to the aircraft, I noticed the left and center IRU lights flashing. The CDU had not yet aligned. After waiting a period of time, I shut down the IRU's and reentered the ramp coordinates and the IRU's aligned properly. A passenger who recognized me told me that he had seen a passenger using a cell phone as we departed bda. Everything appeared normal until we were questioned by ATC.

Title: B757 FLT CREW HAS NAV EQUIP PROBS DEPARTING TXKF.

Narrative: I HAVE BEEN FLYING NORTH ATLANTIC TRIPS WHERE WE HAVE 3 MAN COCKPIT AND 3 MAN XCHKS. THIS IS MY FIRST 2 MAN CREW IN AWHILE. IT IS POSSIBLE SOMETHING WAS MISSED. THIS IS NOT AN ATTEMPT TO MAKE ANY EXCUSES BUT TO TRY AND RESOLVE THIS ISSUE. AFTER FINISHING MY FLT PLANNING IN BDA OPS, I WENT TO THE ACFT. THE FO WAS IN THE COCKPIT AND I ASKED HIM IF ALL WAS WELL WITH THE PREFLT AND THE ACFT. HE SAID YES. HE ALSO SAID THAT HE WAS HAVING TROUBLE UP-LOADING THE FLT PLAN TO THE FMC FROM THE CDU. I MANUALLY LOADED THE FLT PLAN IN THE FMC AND THEN WE BOTH CHKED THE RTE, LEGS AND THE MILEAGE. ALL CHKED OKAY. I THOUGHT AT THAT TIME I CHKED THE NAV DATA LIKE I ALWAYS DO AS REQUIRED. I FEEL SURE I DID. AFTER PUSHING AND STARTING, WE TAXIED TO RWY 12. THIS IS A VERY SHORT TAXI. AFTER COMPLETING TAXI CHKLIST, WE WERE GIVEN CLRNC TO TAKE THE RWY AND TKOF. THE ACFT HEADINGS CHKED AND ALL LOOKED NORMAL. WE WERE LOOKING DIRECTLY INTO THE SUN AT THIS TIME. BOTH OF US WERE TRYING TO ADJUST AND SHADE OUR EYES SOMEWHAT FROM THE DIRECT SUNLIGHT. HE SAID IT WOULD PUT US 60-80 MI OFF COURSE AT GABES. I IMMEDIATELY LOOKED AT ALL 3 IRU'S. ALL 3 HAD SIMILAR DATA. I ASKED IF WE COULD ORBIT UNTIL I LOOKED AT EVERYTHING. I CHKED THE RAW DATA WITH THE VOR ON BDA AND THE FIX PAGE ON THE CDU. I COULD STILL SEE BDA. WHAT I COULD SEE ON THE RAW DATA AND WHAT I SAW LOOKING FROM THE ACFT, WAS DIFFERENT. IT WAS OBVIOUS THAT WE HAD A NAV FAILURE. I CONTACTED BDA OPS AND ASKED THEM TO ADVISE DISPATCH AND MAINT. I THEN ELECTED TO RETURN TO BDA AND DEAL WITH THE PROB ON THE GND. DURING THE APCH AND LNDG, THE ACFT CONTINUED TO SOUND 'PULL-UP' UNTIL AFTER WE CLRED THE RWY AND RETRACTED THE FLAPS. THE ACFT HAD NO IDEA WHERE IT WAS. AFTER WE TAXIED TO THE GATE, I ASKED THE FO HOW HE ENTERED THE NAV DATA TO ALIGN THE IRU'S. HE SAID THAT HE USED NORMAL PROCS. HE THEN SAID HE WENT OUTSIDE TO DO HIS WALKAROUND. SUPPLEMENTAL INFO FROM ACN 599594: WHEN I RETURNED TO THE ACFT, I NOTICED THE L AND CTR IRU LIGHTS FLASHING. THE CDU HAD NOT YET ALIGNED. AFTER WAITING A PERIOD OF TIME, I SHUT DOWN THE IRU'S AND REENTERED THE RAMP COORDINATES AND THE IRU'S ALIGNED PROPERLY. A PAX WHO RECOGNIZED ME TOLD ME THAT HE HAD SEEN A PAX USING A CELL PHONE AS WE DEPARTED BDA. EVERYTHING APPEARED NORMAL UNTIL WE WERE QUESTIONED BY ATC.
