Narrative:
The flight was the last of a 3-DAY pairing, the first of 5 legs. We were notified when we arrived that ZJX was down and dispatch had filed us at 9000 ft and added additional fuel for the flight. We were advised by clearance delivery that ZJX was back up and received the standard company clearance. Fll traffic had significant delays. Fll ground and tower were clearly far busier than normal. It took us approximately 30 mins to reach the departure runway (runway 27R). We were cleared 'on to hold' expecting minimum time on the runway, as traffic was approximately 4-5 mi on final. Within less than 1 min, tower made a nonstandard transmission. Tower apologized for the delay on the ground, provided winds and initial departure heading, but never completed the transmission with 'cleared for takeoff.' with the start of the transmission, the captain added power and began the takeoff run to avoid conflict with traffic on final. By the time I realized the words, 'cleared for takeoff' were not in the transmission, we were accelerating down the runway. I read back the departure heading and 'cleared for takeoff' in the hope of clarification from tower. Tower then transmitted something to the effect of 'you're already rolling.' once airborne, we were switched over to departure without further comment. Mia departure initially denied clearance to the high center altitudes. After a short delay, we were cleared into center airspace and proceeded to our destination. This is what I 'think' happened: as fll tower was in the process of clearing us for takeoff, he received notification from departure control to hold us on the ground due to center saturation. This caused him to stop his takeoff transmission in mid-sentence. He did not take action to stop our takeoff because we were already accelerating down the runway. Supplemental information from acn 605187: we were then cleared onto the runway to hold. 'Traffic on a 5 mi final, be ready.' (I think I heard cleared for takeoff.) the first officer read back, 'heading 270 degrees, 3000 ft cleared for takeoff' and we rolled. Tower then said, 'oh, I see you're on the roll.' I thought nothing of this, as we climbed to 3000 ft. When we contacted mia departure, they informed us ZJX was again not accepting traffic, and she would have to keep us in her airspace. Long story short, they worked it out and we were on our way. Thinking back, tower may have received word from ATC to stop departures and may not have given takeoff clearance. I really don't know. I'll make sure the next time.
