Narrative:
E145 entered adjacent facility sector without handoff. Communication change prior to aircraft entering adjacent sector with no handoff. Receiving control position did not alert xferring facility as to what was occurring until violation took place. ZAU controller was talking to aircraft 4 mins prior to alerting ZOB what was taking place, and only after ZOB called to coordination another flight. The situation could have been avoided 30 mi prior. Contributing factors include: sector volume, complexity, WX deviations in sector, all causal factors along with the receiving controller not doing his part in operating the national airspace system safely. The time period of this error is a very busy time at ZOB with arrs to ord/mdw, dtw/ptk/fnt, cle, pit, and cak -- all scheduled at that time.

Title: OPDEV OCCURRED WHEN ZOB ARTCC CTLR XFERRED AN ACFT TO ZAU CTLR WITHOUT MAKING A HDOF.
Narrative: E145 ENTERED ADJACENT FACILITY SECTOR WITHOUT HDOF. COM CHANGE PRIOR TO ACFT ENTERING ADJACENT SECTOR WITH NO HDOF. RECEIVING CTL POS DID NOT ALERT XFERRING FACILITY AS TO WHAT WAS OCCURRING UNTIL VIOLATION TOOK PLACE. ZAU CTLR WAS TALKING TO ACFT 4 MINS PRIOR TO ALERTING ZOB WHAT WAS TAKING PLACE, AND ONLY AFTER ZOB CALLED TO COORD ANOTHER FLT. THE SIT COULD HAVE BEEN AVOIDED 30 MI PRIOR. CONTRIBUTING FACTORS INCLUDE: SECTOR VOLUME, COMPLEXITY, WX DEVS IN SECTOR, ALL CAUSAL FACTORS ALONG WITH THE RECEIVING CTLR NOT DOING HIS PART IN OPERATING THE NATIONAL AIRSPACE SYS SAFELY. THE TIME PERIOD OF THIS ERROR IS A VERY BUSY TIME AT ZOB WITH ARRS TO ORD/MDW, DTW/PTK/FNT, CLE, PIT, AND CAK -- ALL SCHEDULED AT THAT TIME.
