Narrative:
I have just received notice of a crew oxygen bottle that I supposedly svced, that was safety wired in the closed position. Log page indicates that I svced the crew oxygen for aircraft on jun/tue/04. Log page indicated that on jul/sun/04 (40 days later) the flight crew finally noticed that they didn't have any oxygen for emergency purposes. I will tell you that they were the ones who svced crew oxygen and did not write it up in the log page as well as doing the job correctly. I would even go so far as to say that the crew oxygen bottle has probably been svced at least a half a dozen times since I last svced it and also went unrecorded in the logbook.
