Narrative:
On jul/wed/04, I was summoned to air carrier X chief inspector's office and informed that I was being disciplined for giving the ok to close on an overwing exit service panel for aircraft X. I was told at that time that the bypass pins were left in the slide bottles. When given the ok to install and closure card, I went to the aircraft and saw that the panels were already closed. (This aircraft was in the hangar for a C-1 check.) I was informed that the panel closure had already been complied with on an 'a' check work card. An 'a' check work card had been issued and signed off, I assumed the panel installation had been bought off on the section of the card that states return aircraft to normal condition. It was poor judgement on my part not to have them re-pull the panel and check the condition of the bypass pins. Duplicate cards have been a real issue with air carrier X maintenance.

Title: AN AIRBUS 319 WAS DISPATCHED IN NON COMPLIANCE WITH THE L OVERWING ESCAPE SLIDE INFLATION BOTTLES PINNED. SLIDE RENDERED INOP.
Narrative: ON JUL/WED/04, I WAS SUMMONED TO ACR X CHIEF INSPECTOR'S OFFICE AND INFORMED THAT I WAS BEING DISCIPLINED FOR GIVING THE OK TO CLOSE ON AN OVERWING EXIT SVC PANEL FOR ACFT X. I WAS TOLD AT THAT TIME THAT THE BYPASS PINS WERE LEFT IN THE SLIDE BOTTLES. WHEN GIVEN THE OK TO INSTALL AND CLOSURE CARD, I WENT TO THE ACFT AND SAW THAT THE PANELS WERE ALREADY CLOSED. (THIS ACFT WAS IN THE HANGAR FOR A C-1 CHK.) I WAS INFORMED THAT THE PANEL CLOSURE HAD ALREADY BEEN COMPLIED WITH ON AN 'A' CHK WORK CARD. AN 'A' CHK WORK CARD HAD BEEN ISSUED AND SIGNED OFF, I ASSUMED THE PANEL INSTALLATION HAD BEEN BOUGHT OFF ON THE SECTION OF THE CARD THAT STATES RETURN ACFT TO NORMAL CONDITION.
