Narrative:
Flight was cleared to taxi to runway 36R via bridge route. I completed the taxi checklist after exiting the ramp. Captain did not call for the extension of the flaps and slats at this time. On two previous occasions I have flown with capts who, for unknown reasons, would not call for the flap and slat extension until approaching the runway. I read the before takeoff checklist down to the takeoff briefing/PA and received the proper responses. I did not read or move the toggle for the flaps/slats challenge because captain did not yet call for their extension. We appeared to be #1 for takeoff. At this point, I advised captain that I felt very uncomfortable with this attempted takeoff. He told me that it should not be a problem. Approximately 5 seconds later, I again advised captain in a louder voice that I felt very uncomfortable with this attempted takeoff because of our confign and flaps. Again captain said that it would be no problem. The control tower cleared us into position and hold. I asked captain if he wished to accept this clearance. He replied yes. In a loud voice I told captain that I felt that this attempted takeoff was dangerous because of our confign and the flaps. The captain made the turn to line up on the centerline. The control tower cleared us for takeoff. In a very loud voice I told captain that this attempted takeoff was very dangerous because the MD80 is not certified for takeoff without the flaps and slats being extended. He finally ordered the flaps and slats to be extended. During the takeoff roll and initial climb captain proceeded to berate me, called me various unsavory names and cursed at me. I will not, for safety reasons, accept a future crew pairing assignment with captain due to his level of immaturity, disregard for approved procedures. Supplemental information from acn 629306: I was advised by union safety today that an as soon as possible report had been filed by first officer regarding what he considered to be a safety issue on flight dfw-MMMX on. The point in question seems to be he was concerned we were going to depart with our flaps and slats not extended when in fact that item had not yet been called for on the challenge and response checklist. He made a comment that he was uncomfortable with the takeoff, and I thought this was in response to the flap 6 degree high speed takeoff. He never made any reference to the flap and slat item on the checklist. When I noticed that that item had not yet been completed, I called for flaps 6 degrees and then the remainder of the checklist was completed.

Title: A MISCOM BTWN THE CAPT AND FO OF AN MD80 RESULTED IN AN INCOMPLETE TKOF CHKLIST WITH THE FLAPS AND SLATS IN A RETRACTED AND UP POS.
Narrative: FLT WAS CLRED TO TAXI TO RWY 36R VIA BRIDGE RTE. I COMPLETED THE TAXI CHKLIST AFTER EXITING THE RAMP. CAPT DID NOT CALL FOR THE EXTENSION OF THE FLAPS AND SLATS AT THIS TIME. I READ THE BEFORE TKOF CHKLIST DOWN TO THE TKOF BRIEFING/PA AND RECEIVED THE PROPER RESPONSES. I DID NOT READ OR MOVE THE TOGGLE FOR THE FLAPS/SLATS CHALLENGE BECAUSE CAPT DID NOT YET CALL FOR THEIR EXTENSION. WE APPEARED TO BE #1 FOR TKOF. AT THIS POINT, I ADVISED CAPT THAT I FELT VERY UNCOMFORTABLE WITH THIS ATTEMPTED TKOF. HE TOLD ME THAT IT SHOULD NOT BE A PROB. THE CTL TWR CLRED US INTO POS AND HOLD. I ASKED CAPT IF HE WISHED TO ACCEPT THIS CLRNC. HE REPLIED YES. IN A LOUD VOICE I TOLD CAPT THAT I FELT THAT THIS ATTEMPTED TKOF WAS DANGEROUS BECAUSE OF OUR CONFIGN AND THE FLAPS. THE CAPT MADE THE TURN TO LINE UP ON THE CTRLINE. THE CTL TWR CLRED US FOR TKOF. IN A VERY LOUD VOICE I TOLD CAPT THAT THIS ATTEMPTED TKOF WAS VERY DANGEROUS BECAUSE THE MD80 IS NOT CERTIFIED FOR TKOF WITHOUT THE FLAPS AND SLATS BEING EXTENDED. HE FINALLY ORDERED THE FLAPS AND SLATS TO BE EXTENDED. DURING THE TKOF ROLL AND INITIAL CLB CAPT PROCEEDED TO BERATE ME, CALLED ME VARIOUS UNSAVORY NAMES AND CURSED AT ME. I WILL NOT, FOR SAFETY REASONS, ACCEPT A FUTURE CREW PAIRING ASSIGNMENT WITH CAPT DUE TO HIS LEVEL OF IMMATURITY, DISREGARD FOR APPROVED PROCS. HE MADE A COMMENT THAT HE WAS UNCOMFORTABLE WITH THE TKOF, AND I THOUGHT THIS WAS IN RESPONSE TO THE FLAP 6 DEG HIGH SPD TKOF. HE NEVER MADE ANY REF TO THE FLAP AND SLAT ITEM ON THE CHKLIST. WHEN I NOTICED THAT THAT ITEM HAD NOT YET BEEN COMPLETED, I CALLED FOR FLAPS 6 DEGS AND THEN THE REMAINDER OF THE CHKLIST WAS COMPLETED.
