Air Traffic Incident | Intra Facility Coordination Failure Operational Error |
Narrative:
The flight was a dual IFR end-of-course examination conducted in visual meteorological conditions. WX at the time of the incident was reported (via ATIS) as sky clear, visibility 10 mi, with light winds. The student had control of the aircraft for the duration of the incident. After holding short at intersection B5 to runway 2 (4049 ft by 100 ft), we were cleared for takeoff on runway 2 with no delay (traffic reported on left base and in sight upon takeoff). During the takeoff roll when we were approximately 1000 ft down the runway, a jet-a fuel truck passed from left to right (west to east) on taxiway a, crossing the runway approximately 800 ft in front of us. We were at an appropriate airspeed to rotate, and it did not appear that we would be able to stop before impacting the fuel truck, so the student rotated and began a normal climb out. The fuel truck stopped after crossing the runway in front of us, across the hold short lines on taxiway a. I called the tower over the radio to verify that we had been cleared for takeoff, and the tower responded that we had taken off correctly and that they were talking to the fuel truck on the ground frequency. The remainder of the flight continued without incident.
Original NASA ASRS Text
Title: C172 FLT INSTRUCTOR RECOUNTS CLOSE CALL WITH FUEL TRUCK XING RWY AS ACFT WAS DEPARTING.
Narrative: THE FLT WAS A DUAL IFR END-OF-COURSE EXAMINATION CONDUCTED IN VISUAL METEOROLOGICAL CONDITIONS. WX AT THE TIME OF THE INCIDENT WAS RPTED (VIA ATIS) AS SKY CLR, VISIBILITY 10 MI, WITH LIGHT WINDS. THE STUDENT HAD CTL OF THE ACFT FOR THE DURATION OF THE INCIDENT. AFTER HOLDING SHORT AT INTXN B5 TO RWY 2 (4049 FT BY 100 FT), WE WERE CLRED FOR TKOF ON RWY 2 WITH NO DELAY (TFC RPTED ON L BASE AND IN SIGHT UPON TKOF). DURING THE TKOF ROLL WHEN WE WERE APPROX 1000 FT DOWN THE RWY, A JET-A FUEL TRUCK PASSED FROM L TO R (W TO E) ON TXWY A, XING THE RWY APPROX 800 FT IN FRONT OF US. WE WERE AT AN APPROPRIATE AIRSPD TO ROTATE, AND IT DID NOT APPEAR THAT WE WOULD BE ABLE TO STOP BEFORE IMPACTING THE FUEL TRUCK, SO THE STUDENT ROTATED AND BEGAN A NORMAL CLBOUT. THE FUEL TRUCK STOPPED AFTER XING THE RWY IN FRONT OF US, ACROSS THE HOLD SHORT LINES ON TXWY A. I CALLED THE TWR OVER THE RADIO TO VERIFY THAT WE HAD BEEN CLRED FOR TKOF, AND THE TWR RESPONDED THAT WE HAD TAKEN OFF CORRECTLY AND THAT THEY WERE TALKING TO THE FUEL TRUCK ON THE GND FREQ. THE REMAINDER OF THE FLT CONTINUED WITHOUT INCIDENT.
