Narrative:

Several operrors of omission and commission resulting from accrued fatigue. Micro-sleeps suspected. All errors in this case were minor and were caught within the cockpit. Exact sequence flown several times but in this case, seat next to crew rest seat was sold to revenue passenger who engaged in loud conversation and disruptive movement during entire break. No rest obtained resulting in time since waking approaching 21 hours as errors began to occur.

Title: ACCRUED FATIGUE AND MICRO-SLEEPS ARE RPTED BY A B767 PLT AS THE SUSPECTED CAUSE OF MINOR OMISSIONS AND ERRORS MADE DURING AN EXTENDED RANGE FLT.

Narrative: SEVERAL OPERRORS OF OMISSION AND COMMISSION RESULTING FROM ACCRUED FATIGUE. MICRO-SLEEPS SUSPECTED. ALL ERRORS IN THIS CASE WERE MINOR AND WERE CAUGHT WITHIN THE COCKPIT. EXACT SEQUENCE FLOWN SEVERAL TIMES BUT IN THIS CASE, SEAT NEXT TO CREW REST SEAT WAS SOLD TO REVENUE PAX WHO ENGAGED IN LOUD CONVERSATION AND DISRUPTIVE MOVEMENT DURING ENTIRE BREAK. NO REST OBTAINED RESULTING IN TIME SINCE WAKING APCHING 21 HRS AS ERRORS BEGAN TO OCCUR.
